Left Testicle soft and lumpy, please help

I have masturbated for 3 years regularly and doing that make my penis tilt to left side and my left testicle hang lower than right and also smaller than right one. Left testicle is also soft and itching occurs on the scrotum.

With the above problem, hairfall (dandruff wet nature) is also going along with falling eyesight. Dribbling of urine after urination is happening for past 3 years. I have consulted a physician for my hairfall problem and first he gave me biotin and multivitamins for 3 months and then he gave me android capsultes(testosterone) but i stopped his medicines due to longer intake period of allopathic medicines.

Now, my condiition is, my hairs are dry but scalp is wet with dandruff. Dribbling of urine is continuous. Eyesight is weakening rapidly. Left testicle is soft and hang lower than right and also smaller than right one and itching occurs on the scrotum.
