Hissing and Whistling in Ears

I had been hit by Football on my face( on right side of face) around 30 years ago.
Since then I am experiencing 'HISSING and WHISTLING' noise in both ears, more prominently in right ear.
I have tried Arnica 1M, and Phosphorous 30 but no relief was found.
I have 'Hypertension' since 5 years. Earlier it was 130/90 and now it is 150/110.
Not tried any allopathic medicine. Addicted to smoking (4/5 per day).
Pls suggest any remedy for me.

Regards,
