Pilonidal Sinus - 27 Yr Male

Dear Doctors,

At the outset, let me thank you all for the guidance and hope provided in the forums here.

I have been suffering from Pilonidal Sinus for few months now and realised the condition just recently. Major Surgery was suggested but I did not want to risk it since I have already been operated upon.

Please answer the following questions in a descriptive manner after careful analysis and recollection of previous experiences and happenings.

1. Describe your main suffering?

Pilonidal Sinus

2. What other physical sufferings do you have in your body?

Related to the above problem, it is painful and sore at times and discharges blood and pus and stains the undergarments. At times, the lump gets bigger and smaller.

3. What mental sufferings / feelings do you have associated with your physical sufferings?

Not much but I feel irritated and helpless when I can’t do things normally (such as sitting properly, etc.). It has been few months that I slept on my back since it pains. I have been sleeping on my sides mostly. It has been causing a bit of backpain.

4. What exactly do you feel when you are at your worst?

helpless and inconvenient.

5. When did it all start? Can you connect it to any past event or disease?

It has been there since 9 months if not more. It started as a harmless lump and I ignored it until a day it got infected and it was painful and bleeding. When I consulted a general practitioner, he said it’s a sebaceous cyst and advised it can be surgically removed without any issue. Since he mentioned it had to be removed only when it is completely painless, I waited in vain and went to him after nearly two months saying it’s not getting painless at all. After a few antibiotics for a few days, it was relatively painless and he performed the surgery to remove the so-called sebaceous cyst (only later we all discovered that it was no cyst at all but a passage formed by Pilonidal Sinus). There were three stitches and the surgery was complicated than thought and there was a lot of bleeding. After an inconvenient one week, he removed the stitches. Surprisingly it came back again promptly. This time, he was confused and referred me to a colorectal surgeon

The colorectal surgeon confirmed that this was indeed the pilonidal sinus and the lump was due to the pilonidal sinus. He suggested I need to go for a major surgery where he would have to cut 8cmx8cm and 20-30% chance of recurrence. He also mentioned I would have to be on bed for 3 weeks. I did not want to take the chance and have been living with it ever since (over 2 months now).

6. Which time of the day you are worst?

In the night, when I sleep.

7. What are the things which aggravate your suffering and which are those which ameliorate the same?

Not sure..

8. Do your think your sufferings have relation to any external stimuli (like, change of place) or any internal biological changes in the body, like, menses (in females)?

Don’t know

9. When do you feel better, during hot weather or cold weather, humid or dry weather?

cold weather I think.

10. Describe your general mental set up? Are you Moody, Arrogant, Mild, Agreeable Changeable, Nervous, Suspicious, Easily offended, Quiet, Arguing, Irritating, Lazy etc.

- How do you feel before or during a thunderstorm?

Agitated

- Do you like being consoled during your tough times? No


- Are you sensitive to external stimuli like smell, noise, light etc? Yes, a bit

- Do you have any typical habit or gesture like nail biting, causeless
weeping, talking to one self etc? Yes, I tend to talk to myself

- How do you feel about your friends, family, your children and especially your husband / wife? Close.

11. What are your fears and do you dream of any situation repeatedly? No.


12. What do you crave for in food items and what are your aversions?

No special preferences really

13. How is your thirst: Less, Normal or Excessive?
Normal


14. How if your hunger: Less, Normal or Excessive?
Less

15. Is there any kind of food which your body can’t stand? Not sure

16. Is your sweat normal or less or more? Where does it sweat more: Head, Trunk or Limbs? Normal

17. How is your bowel movement and stool type? Ok

18. How well do you sleep? Do you have a particular posture of sleeping?
I don’t sleep well.

19. Do you think you are able to satisfy your sexual desires in general?
I am not married.

20. How do you think you are different from others, if at all?
My thoughts are completely different from others. I find it difficult to mingle with people and participate in discussions.

21. What medications have been taken earlier by you to treat the diseases and do you have any particular symptom surfacing after the medication?
Nothing particular really

22. What major diseases are running in your family?
Nothing that I know of

23. Describe, how do you look like? Describe your overall appearance

I look older than my age – especially these days, I have a pot belly and look a little hefty. I am around 5’6” tall and weight around 65 kg. My waist is 33-34. My hair is falling rapidly and hairline is receding.

Based on my browsing through ALL the posts related to sinus and replies from doctors (especially from Dr.Pankaj Varma and kadwa), I have summarised it as follows.

Take Myristica 200..one dose for three consecutive days in the morning on empty stomach.

No other medicine on these days.

On 4th day start wit h Silicea 6c..three times a day...continue for two weeks.

These meds will promote the pus to come out quickly and will stop the system from making further pus.

Externally clean the area and apply Calendula ointment ...three times a day.

take silicea 200c thrice a day to wind up the treatment.

Kindly help check if the above is OK. I am currently staying in Malaysia and have arranged for the following medicines through a friend coming over.

Myristica 200c
Silicea 6 or Silicea 30
Silicea 200
Calendula ointment
Calcarea Sulph 6X

If the medicine is in pills form 4 pills. Don't touch pills with hand. Use cap of bottle to take pills.
If the medicine is in liquid dilution form, 3-4 drops in some 20 ml water. Sip up slowly.

If it is smaller (tiny)...then 8 or 9
If bigger than a sugar crystal.....3

Also need to follow homeo restrictions like no coffee, no raw onion/garlic, no strong perfumes, don't eat or drink anything within 30 minutes before or after taking medicine.

Thank you for the prompt response.

Please find my response here for your question as well as the other questionnaire. I tried my best to answer them.

Why do you think that you cannot mix with people? Do you fear them? Do you have a superiority complex? When did it started? whether it started after any particular event?

It's not exactly superiority complex but I feel isolated because I feel that none of the people around me share the same passion as I do - whether it's books, music, movies, religion, success in general, etiquette, etc. This makes it difficult for me to connect to them and engage in a healthy discussion.

No particular event but I was shy and inferior as a child but not much now (even though not completely gone). Perhaps this would have triggered this..


Other questions

1. Describe your main sufferings and other related or unrelated sufferings in your own words with exact sensations, locations, modalities and probable causes.

As of now, major suffering is this Pilonidal Sinus, which I was told is due to hair being trapped up in the skin. Actual sinus is in the natal cleft but I have a boil/lump near the natal cleft on the left hip, where I was told the sinus is discharging pus/blood,etc. Currently, it is a little painful and I see that my undergarments are stained.

Other problems I think are laziness, which is purely due to my lack of exercise and sitting on the chair for immensely long amount of time, I guess


2. Write an essay on yourself as a person. Describe your nature, likes and dislikes, fears and dreams, your ambition in life, your inner-most desires, your place in society etc.

I am a self-confident and mature person, always want to achieve many things in life. I am a peace loving person and would like to see everything go according to plan but in case anything goes wrong or not according to plan, I feel restless and anxious. I have big ambitions in life – to do my MBA from one of the major B.School, to do my Ph.D.

At times, I am moody and restless and may get tired soon but if I am at any task that I am interested in, I will be engrossed in it. I am passionate about literature, movies, music. I read a lot and inclined towards religion and spirituality. I believe in God and dream of a wonderful world. I hate negativities in life.

I want to be a responsible citizen and family member. I feel that my foremost duty is to make my family happy and I always strive to earn respect.


3. What is your profession? Do you enjoy yourself at work? Is it a profession you have willingly chosen? If not, what would it be as per your choice?

IT. There are a few tasks I enjoy but not entirely. This was not willingly chosen but I guess it’s more due to herd mentality and family, etc. Not sure which would have been my choice but I have strong affinity towards learning, knowledge and teaching and university life – so maybe, I would have liked to be a professor..


4. What would you like to change in your personality, if at all?

My height – would have liked to be a little taller
My belly – would like a flat stomach
Face and Hair – would like a more soft and spotless face and a good head of hair (right now, it is a little bald)

All in all, I would like to look my original age (27) but I usually look much older 

Not really but at times, I prefer to be lonely. I prefer solitude, I guess.


8. Do you get angry often? If not, do you feel the anger inside at least? What are the things / issues on which you get angry the most?

May not be anger but at times, I always feel that I can be much better than what I am and that I am not striving enough for that – whether physically, mentally, financially, etc. I tend to be angry when I see people not following rules or behaving arrogantly and rudely.

9. Did you have any issues regarding your parenting by your guardians?

No. I have had a very comfortable childhood and I have always been proud of my family.

Please follow homeo restrictions like no coffee, no raw onion/garlic, no strong perfumes, don't eat or drink anything within 30 minutes before or after taking medicine.

I am sorry but I don't have the medicine arsenicum album 200c with me.

I just requested for the following which will be ready in a day or two :(

Kindly let me know if there are any alternatives. Any places I can get this particular medicine in Malaysia/Singapore?

I am sorry but if I have known earlier, I would have arranged for this arsenicum album 200c as well

Please do the breathing exercises regularly. As you have read some of my earlier posts i assume that you have also read about the breathing exercises.
Don't worry if you can start your treatment after 2 days or 4 days.
There is a difference beween x and c potency. i have advised you to take silicea 6x.

May I check with you about 'arsenicum album 200c', what will this do?

Is 'arsenicum album 200c' an alternative to Myristica? I gather that Myristica is to cut open the wound. Does arsenicum album work the same way? If yes, can I take Myristica instead (I have it ready with me and it may take at least 3 weeks for me to get arsenicum album from India..)

1. Instead of Silicea 6, I got Silicea 30. Is it OK?

2. Is it not recommended to use Myristica 200 at all? If yes, I will need to source for arsenicum album 200 in 3-4 days.

The treatment plan including ars alb 200 - silicea 6x is an individulaized treatment plan for you. It will proceed further and may require more remedies down the line depending on the symptoms. The treatment plans containing myristica 200 - silicea 6c - silicea 200c or another plan that may contain calc suplh 6x and haridra khanda are broadband plans that don't need individualization. It is better to start with the individualized plan as you have given the necessary information.

I will start the plan tomorrow and keep you posted.

Meanwhile, I would appreciate if you can let me know in advance about any further medicines that may be required.

Just to rewind, I think I have pilonidal sinus as well as a cyst/abscess.

Not much difference so far but itching sensation on the lump..

The lump has become a little larger and is a bit painful.

Kindly let me know what should I look forward to? I mean any changes, etc.

Also, I have stopped taking alcohol and non-vegetarian. Does this matter?

i congratulate you for having effected changes in your life-style. Please incorporate breathing exercise in your life-style.
Silicea is expected to drain the lump. It is better to report after a gap of atleast 3 days, otherwise you will press for change in remedy.

Thank you for the encouragement.

It's been 4 days I have taken Silicea 6x. On the 2nd day, there has been a bit of blood and puss coming out and not much difference after that.

When I feel the lump, it has become a little dry. Shall I continue with Silicea 6x?

Any other suggestions?

But today morning, after I woke up, found that the lump has become larger and is sore and hurting..

Meanwhile, just finished with Day 7 of taking Silicea 6x

Today, as mentioned earlier, in the morning the lump was larger and painful and there has been quite a bit of discharge of blood (as seen by the stains on clothes). As of now, it is still sore.

Also, apart from the Pilonidal Sinus issue, I have found out another issue from the past one week or so.

Heel Pain - There is a shooting pain in my ankles - especially when I wake up in the morning and sit for sometime and get up after that. It is just in the heels.

Is this in anyway related to the Pilonidal Sinus or anything?

Anything that I can do for this, kindly advise.
