I would like to know if anyone can help me with the PMDD I suffer from. If I fill out the questionnaire, will someone follow up? Thank you

This is just a forum. Assume posts are not from medical professionals.
Here is the completed questionnaire and I hope to hear back.


These questions are designed to get the following information from the patient
1. Mental State of the patient
2. Physical Ailments
3. The likely cause for above problems
4. The modalities like whether the patient feels well or worse in hot weather, cold weather etc., he is relieved by / worsenened by hot applications, cold applications etc.

i also thank Rishimba for designing such wonderful tool for case taking. i can also understand the hard work that he has done for desiging these questions.

Patients can use this questionnaire for submitting their cases. The effectiveness of remedy selection is directly proportional to the details provided by the patient while replying these questions.

Please answer the following questions in a descriptive manner after careful analysis and recollection of previous experiences and happenings.

1. Describe your main suffering?

Very irritable and moody 2 weeks before period. Could care less if I got divorced and generally feel like I don’t want husband to touch me. Irritable to kids too, but still affectionate and caring with them. Have low self confidence and don’t care to leave the house at this time. As soon as period starts I am myself again- happy and sometimes moody and irritable but generally fun to be with. I also get emotionally attached at this time and feel very in love with my husband. However, if he is too nice to me, I find I can be mean to him and vice versa- if he is moody, I’ll try harder to please him.

2. What other physical sufferings do you have in your body?
migraines and stomach aches

3. What mental sufferings / feelings do you have associated with your physical sufferings?
I do sometimes feel like I am going crazy Also I do not express my emotions very easily although I am not a closed book. I find it easier to tell a friend that she/he looks nice rather than my husband or family.

4. What exactly do you feel when you are at your worst?
I have felt that it wouldn’t be so bad if I died



6. Which time of the day you are worst? All about the same

7. What are the things which aggravate your suffering and which are those which ameliorate the same?

Stress aggravates the irritability. For instance, if a bill comes in that is wrong I have to fix it right away- I can’t wait and I will be irritable until it gets done. The start of my period usually takes the suffering away. Going to do something really fun like an adventure park or spa makes me feel better. Having to go somewhere unknown when I’m suffering pmdd is hard to do and I get irritable from that.

8. Do your think your sufferings have relation to any external stimuli (like, change of place) or any internal biological changes in the body, like, menses (in females)?
I know my migraines started when I was planning my wedding and got really bad when I moved to a hot place many miles away from where I had lived for 20 years.
I never really gave much thought to my pmdd until after I had kid so I don’t know if anything triggered that
9. When do you feel better, during hot weather or cold weather, humid or dry weather?
I do not function well in hot, sunny weather- I get migraines and generally don’t care to leave the house, menses or not. I do enjoy a cloudy day very much.


10. Describe your general mental set up? Are you Moody, Arrogant, Mild, Agreeable Changeable, Nervous, Suspicious, Easily offended, Quiet, Arguing, Irritating, Lazy etc.
Changeable, agreeable, nervous, anxious, suffer panic attacks. I like to be right. I won’t express my opinion unless I’m comfortable with the people. When I drive I do constantly look in my rearview to make sure that I’m not holding anyone up so to speak. I would have a hard time exploring a new place by myself or trying something new alone, in fact, I probably wouldn’t do it. I took a long road trip and when we entered a really busy highway I guess I freaked out and next thing I know I was dizzy and felt faint and was shaking and immediately got off the highway. Since then, I have had mild panic and anxiety just from driving locally so I try to breathe and meditate to help myself. I have a quick temper and can throw things. I get jealous easy too. I don’t cry often even though sometimes I feel like I need to and I do feel better after I do. I get a headache if I mentally overwork myself and a stomach ache if I emotionally overwork myself. I think I am very impressionable also. I get excited easily if I am looking forward to a fun event. I don’t like sitting around doing nothing and usually have things planned every weekend.
- How do you feel before or during a thunderstorm?
No different

- Do you like being consoled during your tough times? I act like I don’t but I do
- Are you sensitive to external stimuli like smell, noise, light etc? very sensitive to smells and light, fluorescent light gives me a headache and makes me feel dizzy

- Do you have any typical habit or gesture like nail biting, causeless
weeping, talking to one self etc? I talk to myself and make up stories in my head when I am doing busywork. I am restless most of the time and am constantly doing things like cleaning or picking up after the kids or just whatever. I used to blink my eyes very frequently when I was a child and made a weird throat clearing sound when I was a child also.

- How do you feel about your friends, family, your children and especially your husband / wife? My children are very important to my well being and 2 weeks out of the month so is my husband. I talk to my extended family once or twice a month. I love them, but don’t need to talk with them all the time. I make friends easily and find that I am always offering to help them at the slightest notion that they may need it. I find it important to try and help a friend in crisis and wish I could do more to help people in general.

11. What are your fears and do you dream of any situation repeatedly?
I dream of famous people a lot, dream that I lose my teeth, dream of sex,and I used to dream frequently about tidal waves engulfing me but don’t remember if that started before or after I nearly drowned as a child. When I was little(about 4) I remember being deathly afraid of the dark and ghosts but my parents lulled it aside. As an adult I fear the dark sometimes as I think I see thing. I won’t watch a scary movie before bed or I’ll be sure to dream of it or think of it before sleeping. I don’t like heights, but do like roller coasters, and definitely looking down from a high place makes me sick to my stomach.

12. What do you crave for in food items and what are your aversions?
I tolerate most foods. I do like sweets. Too much coffee can make me sick and when I smoked, sometimes I would just feel sick from cigarettes.

13. How is your thirst: Less, Normal or Excessive? normal

14. How if your hunger: Less, Normal or Excessive?
I can eat a lot and think about food alot
15. Is there any kind of food which your body can’t stand?
no
16. Is your sweat normal or less or more? Where does it sweat more: Head, Trunk or Limbs?
normal I guess. I sweat profusely when my emotions are high-like going somewhere new or going to a crowded place which I don’t care for at all
17. How is your bowel movement and stool type?
if I don’t eat good, I get constipated easily
18. How well do you sleep? Do you have a particular posture of sleeping?
I sleep well unless its hot. I used to sleep on my stomach but mostly on my side now and back.

19. Do you think you are able to satisfy your sexual desires in general? Yes but I wish I could tell my husband what excites me without feeling so self conscious. Certain times of the month I am more assertive sexually and other times its hard for me just to close my eyes to enjoy the moment without feeling self conscious. I have no problems satisfying myself.

20. How do you think you are different from others, if at all?

I enjoy mystical and spiritual things. I love tarot and astrology whether its beneficial or not. I suffer panic attacks which have caused me to pass out (crowded, hot rooms).
21. What medications have been taken earlier by you to treat the diseases and do you have any particular symptom surfacing after the medication?
I took Zoloft for depression which was caused by birth control but didn’t like the weight gain so I stopped

22. What major diseases are running in your family?
heart disease, high cholesterol and one uncle had schizophrenia

23. Describe, how do you look like? Describe your overall appearance
5’6, long brown hair, brown eyes, 145 lbs, generally in good shape from various exercise, attractive, mostly proportional in the upper and lower body

If you are not having normal menstrual cycles, please answer the following questions:

- Are the periods early, regular or late in general? How long do they last?
- Do you suffer from any kind of physical or mental discomfort before, during or after the periods?
- Is the flow scanty, normal or excessive? normal
- Is the blood thick bright red or pale watery? Thick bright red
- Do you notice any clots in the flow?not usually

Please take Pulsatilla 200c thrice a day at a gap of 4 hours for only one day (not daily) and report back after 15 days.

One dose means
If the medicine is in pills form 4 pills. Don't touch pills with hand. Use cap of bottle to take pills.
If the medicine is in liquid dilution form, 3-4 drops in some 20 ml water. Sip up slowly.

Please follow homeo restrictions like no coffee, no raw onion/garlic, no strong perfumes, don't eat or drink anything within 30 minutes before or after taking medicine.
