Sex Problem Urgent

Hi to all doctors,

I am 24 years old.
From India.
my weight is 70kg,
height : 5'9'.
Smoker
Blood Pressure is Normal.

I am planning to get married 1 or 2 month(family decided). But Early ejaculation is Big problem.premature ejaculation about 15-20 seconds, also have small penis, penis size 4' and thin, curved under left side. I started masturbation at the age of 15-16. Now stop it.

when i thinking girls or watch erotic movie, crystal water come out in penis but semen like cough and slight smell. when i do some work, i feel very tired. i feel my memory is low.

please advise me how can i fully well within one or two month.
