Tiredness

As student I have to prescribe a case of a man, 40 years old, tall (1.87m) and slim (75 kg), electronic engineer, intelectual person, with the following symptoms:

• Tiredness during the day. After 7pm until midnight or later, he is always fine.
• Needs to sleep 10 hours a day (0 AM to 10 AM). Usually get up at 10 am but when he gets up early, sleepy from 4 pm to 7pm
• Hard to get up in the morning (although he sleeps very well), but after a while he gets better.
• Responsible, always busy, he is worried about everything, sensitive to arguments, always wants to do things in his own way.
• Eat and walk very fast.
• Don´t like to go to parties, or having association with many people, prefers to stay alone or with only one friend.
• Need to have the window of his room open
• Sensitive to noises.
• Lower lids always swollen or red. Red face
• Cold hands, nose, and ears in winter.
• Varicose veins in arms and feet.
• Flatulence: sometimes after eating.
• Hard to keep standing, he likes to sit or lie down.
• Baldness, falling of hair
• Liver: Gilbert´s syndrome, which causes high indirect bilirubin (jaundice)
• Cholesterol: high (200)
• Better in mild and dry weather, worse in hot and cold wet weather -> sulphur
• Better: hot food, window opened, to stay alone.
• Worse: noises, many people, cold and hot weather
• Better: mild weather
• Desires: stay alone, reading the Bible, watching movies, biking, running (not anymore as he feels tired)


According to these symptoms, it seems to me that the medication should be Sulphur or Lycopodium, but I am not sure which one might be the best for this case. My confusion are in the following symptoms:

Tiredness:
For one side is hard for him to get up in the morning , he usually get up late and tired (10 Am) although he sleeps 10 hours, but after he gets up he feels better. Considering this symptom I would say Sulphur is the remedy. Nevertheless, when he must get up in the morning, he gets sleepy in the afternoon (from 4 to 7 pm) and needs to take a nap, so the option could be Lycopodium.
At night, after 7pm he is always well until he goes to sleep (between 11 am and 2 am) -> Lyc or Sulph

Liver
Gilbert´s syndrome. This disease causes a raise in the indirect bilirubin levels, that is why it produces some jaundice. Considering that the liver is involved I should consider Lycopodium.

Mind
I see symptoms of both Sulphur and Lycopodium.
He is an intelligent man, who used to have very good scores when he studied in the university. He is an electronic engineer. Although an intelligent man, he has never progressed in his job as he likes to work alone, and do not like to associate with his workmates. He is not a man who likes parties, he prefers to , be alone, or in the company of only one friend. Probably a proud man who think is better than his workmates. A religious man.
These symptoms call for Sulphur.
Nevertheless, at the same time I see symptoms of Lycopodium in his personality: aversion to accept new responsibilities, little things annoy, hurry when eating, extremely sensitive to correction.

Digestion, sometimes he has flatulences after eating -> Lyc

My conclusion is to start with Sulphur 30c and see how evolves. Lycopodium as the 2nd remedy

This is just a forum. Assume posts are not from medical professionals.
It is the state of the patient that decides the remedy, not the symptoms. You will only remain confused by staying at the superficial level of the symptoms. What is the state that lies underneath all this?

Have you been taught yet to ascertain the state? I know that in teaching my own students, I leave this until they feel more comfortable with simply rubrics.

More complete case notes might be helpful here. By looking at the way he describes himself, his problems, his life, you should be able to see the difference between the two remedies. Symptoms can be misleading when just taken out of a case. Was this a live case or were you just given the case on paper - it is hard to get the state from paper cases, and I would think then that your teacher is just looking for you to find rubrics.

The other thing you need to do is prioritize the symptoms. Place them into the correct order, according to the hierarchy of the body/mind. The remedy that is scoring best at the top of the list, will be more likely to cure than a remedy that has more symptoms at the bottom.

General order that symptoms should be placed in is

Mental
Emotional
General
Local
Pathological

However, peculiarity always trumps these positions. A peculiar General will always be more important than a common Mental. In fact you should eliminate all common (explainable) symptoms from your analysis anyway. It will also reduce your confusion. Just having a few peculiar symptoms, especially in the General level or above, is a much better recipe for a successful prescription than just lumping everything they say together.

I don't really know what rubrics you have used, so it is hard to see what you have taken notice of in the case specifically, how you have translated it, and what order of importance you have placed them in.
