Please make a new post. ( reason is bc of my reply, people will think the case is taken.)
On the new post- go over to homeopathy and more.com and give the info
on that intake form on the new post. Also talk about when this phobia began
and what was going on with you at that time.
This way the needed information will be on the new post to prescribe for
you correctly.
