chronic urticaria and anxiety

Each symptom in as much detail as possible. For example, for a headache, describe the type of pain (ie sharp, dull, stabbing, pulsating etc etc)

Symptom: Chronic urticaria
Where each symptom is located (ie at right temples)
Location: Primarily on the arms & legs. Complains that skin is extremely itchy; yet when I ask "how itchy it is now", I never get over a 3/10 (10 = worse) and skin has only a few very small (1-3cm blotches - not raised, no exudate)mental symptoms seem out of proportion to physical symptoms.

What first started it off (ie walking in a cold wind)
Starts off as red blotch after something has been pressing against it (like a when carrying a large box)

When it's worse (ie on seeing bright lights, on stepping, before period, etc)
Worse after a shower, Worse with stress, Worse with scratching

When it feels better (ie lying down in a dark room, when warm, when cold)
Not sure?
Your sleep pattern and details of any recurring dreams.
What you are sensitive to (ie, heat, cold, touch, criticism, etc)

Your state of mind (fears, anxieties, attitudes, moods, etc)

Any desires or aversions for particular foods, and wether certain foods make you feel better or worse.
can't tolerate milk; although likes ice cream (causes stomach pain)
Details of any major diseases suffered in past.
Very sensitive stomach; many food allergies/intolerances - dairy, wheat.
