Severe lower back pain

I am a 68 years old man and I have been sufferin from severe lower back pain since the last 30 years. The pain is not continuous but occurs inermittently.
The location of the pain is the lower right side of my back.
when it persists for longer periods, it extends to lower hip joint and left knee joint also.
The pain, when it occurs happens with any movement and is very severe at midnight, after waking up from sleep, long hours in seated position, after standing for sometime, coughing, sneezing, any sudden movemenst and jerks, etc.

Relief is only through hot water bag / hot compress.

Please advise how i can get relief from this

Thank You,

Regards,
