Help to stop a cat from fur mowing

It is the same cat formerly with bad diarrhea and rectal prolapse that Arsenicum Album helped a lot that now is itching so badly (or just likes to mutilate himself) that he spends all day licking his lower sides and the base of his tail until the fur it gone and it bleeds. I've noticed if he is distracted, he'll stop so I'm not sure if his skin really itches or what?

Please help. He may have a flea allergy but he licks and bites where there are no fleas. I've tried combination remedies for 'skin' and 'allergies' and neither worked. I gave Rhus Tox days ago and I'm not sure if that did anything for him.
