Premature Ejaculation with Incomplete erection

I started a new relationship this month after 3 years of inactive physical relationship and intercourse. Since Last two weeks, I have been experiencing continuous semen dribbling when touching and kissing my girlfriend. Even to the extent that imaginations and lustful thoughts result in leakage, after a slight erection and normalisation . Though only once we tried to have intercourse, but in both the attempts I failed to achieve proper erection and ejaculated immediately.

My sexual life remained inactive for nearly 3 years, however I relied on masterbation, maximum 02 times during the week. Sometimes would completely stop it for a month. And since last month, I have not been doing this at all. In my previous relationships, I sometimes did have the feeling of premature ejaculaiton, but other times would be satisfactory too. Also I would always achieve good erections quickly, leading to 2 to 3 times of intercourse almost every night. Though the first erection would yield quick ejaculation, but the second and third would be satisfactory and sometimes extraordinary.

However now I have this inabilitiy to even achieve complete erection, not even upto 50% of what i used to achieve earlier and I feel very weak. I need immediate help for premature ejaculation, semen dribbling and weak erections.

Have had lot of worries and anxieties about life in last three years but for last two weeks have been mostly worried about my sex life.
