over masturbation help

Sir,i am a chronic masturbator.i am24 years old I am doing it since last 10 years.in the beginning, i used to do it4 times a day. Initially,i did not know the ill effects of it.but since 3 years,i am feeling the problems caused by it.
some of them are
1. Premature ejaculation
2.erectile dysfunction
3.leaking penis
4.weakness in penis, pain
5. Performance anxiety
etc.
sir, i have tried so many things to overcome the ill effects of habbit, but i failed. I took shilajit and ashwagandha.. Now a days,iam taking neo tablet for premature ejaculation.buti didn't leave to masturbate. I live in depression. I want to be fine as soon as possible. I can do anything.. Plz help me. Plz,tell me the perfect treatment that is available in India. I want to get my sex life back.i don't have any medical history. I smoke seldom. I don't drink alcohol, tea,coffee. I try to quit masturbation, but every time, i failed.. My hair have become thin..no more energy in my body. Sir,i am not expecting your drug to recover me overnight but it should cure me completely.

Please post only one thread for a particular ailment. Posting multiple threads for the same disease will create confusions and u'll not get proper medication.
