Ah this is a sad thing to hear. I will do what I can - with the limitations on working through the forum, I am not sure how much use I will be, but I will certainly try.

Homoeopaths are difficult patients, probably the worst. We know too much, we speak in rubrics, we tend to steer the conversation towards remedies we know (whether we mean to or not). They are often the hardest cases I see in clinic.

By the way, what remedies and what potencies has he had in the last 2 years, and what effects did they have?

This is an extremely difficult case. I am not sure how comfortable I feel treating it through a forum like this.

There are a enormous number of symptoms here. I am not sure I can get any kind of genuine feel for the totality of such a case through written words.

It looks like Kali-brom, and it also looks like Lachesis. I think I would lean more towards Lachesis based on pathology (seems quite syphilitic to me) and your own observation of him (the loquacity, jumping from subject to subject).

However there are so many symptoms and expressions, especially it is hard to get an idea of which direction to go, or what to pay attention to. I am not sure how to proceed with such a case without seeing the person face-to-face.

Thanks for your kind reply and sorry for the delayed response.

Yes, you are very much right, homeopaths are difficult to treat. I communicated the remedies mentioned by you, however, Dr. Ismail gave me some more symptoms and wants you to kindly re-visit his case. Kindly find below what he said.

I have 90% of physical symptoms of Agaricus and bovista, since I have onion like smell in the armpits and deep impression on body if I am lying on the bed or put my hand on sofa or something like that. Used both the remedies, however, no relief from the said remedies.

I also used phophorus 1m, which gave me shine like in my eyes and I thought its going to bring back my eyesight, however, nothing happened.

I greet people by holding their hands strongly or kissing their forehead.

Lachesis is at the stage of inflammation, however, this stage is passed away and now I have yellow discoloration of optic nerve (as mentioned by the physician).

Observers Note:
Dr. Ismail talk too much, and I found him very talkative, talking about his achievements in the field of homeopathy, however, he keeps hiding things especially the remedies names, mainly because he is concerned that if the patient would know the remedy name, then his income would suffer. I also found him running for the money because most of times I found him charging the patient more than his regular fees because he thought that if the patient is paying high fee to allopaths, then why can't he charge a fee as high as a allopath do and there were some cases which he returned un-treated because the person was not giving him his demanded fee. On the other hand he even treated people charging them zero fee where he thought that the person cannot bear the fee. So it was extremes on both sides - charging high and charging zero fee.

One more very important symptom, he cannot tolerate the fan air directly falling on his head. He gets flu like symptoms when he has the fan air direct on the his head. However, fresh air doesn't seem to create any such symptoms.

See that makes it worse. Now he wants to add in symptoms of Agaricus and Bovista. There are too many symptoms in this case - no one remedy covers them. I can only presume either his knowledge of materia medica is polluting the description of his case, or he has used too many remedies over his life.

I based Lachesis on the mentals, and on the pathology. I don't understand referring to Lachesis as 'the stage of inflammation'. Every remedy has inflammation - and his remedy should cover every stage of the disease.

How to make sense of such a complicated case without any real direction? This is like a case a student has put together from a number of books - no logic to it when seen purely as words on the page.

Is he currently taking medicine of any kind?

He is desperate to get his eyesight back and at the same time he has lost his faith on any doctor. He has a strong delusion that he is under some unforeseen forces called 'black magic'.

And you are right, he is putting too many symptoms and mixing things together.

Therefore, I put my own notes so his mentals could be clear to you. If you need further information, I would be glad to give or you think if lachesis is his medicine then please let me know the potency to be used, so I can communicate it.

That symptom of being under a spell is a specific peculiar for Lachesis, and is actually what decided me. I would start with 12c or even better would be 0/1. LMs would definitely be my preference in a case like this. Daily doses with the normal attention to aggravation or new symptoms.

As an aside, there was another unusual symptom which Lachesis covered, which was 'Sensation as if a ball rose from the stomach to the throat'.

However, if I look at that rubric 'Atrophy of the optic nerve' Lachesis is there, but so are a few other possibly useful remedies.

One that stands out to me is Plumbum metallicum.

In fact, when I look at these small rubrics Plb appears where Lach does not:

Erections too easy
Delusion pursued by police
Delusion will be arrested
Fear of undertaking a new enterprise
Desires mutton

I would definitely consider this remedy as well. Only one at time though - so perhaps get hold of Plb 0/1 as well.

Dr. Ismail is more convinced with the PLB, however, LM potency is not available in Pakistan. I would really appreciate if you could give the method to prepare it and dosage, so the same could be communicated to Dr. Ismail.

Alright it needs to be low, so perhaps 12c would be best. Make a liquid dose (3 pillules dissolved into a small bottle with a dropper, 1 part alcohol to 5 parts water).

For each dose hit the bottle 5 times. Place 3 drops into 100mls of water. Stir thoroughly and take 2 teaspoons into the mouth. Hold for 20 seconds and swallow.

Do this once each day for 7 days, and let me know what kind of reaction he has. Obviously he should stop if there is any marked aggravation or development of new symptoms.

Yes you could start with 6c. I would change the dosing to twice a day.
