Severe Problem.. Need help Please..

Hi to All the Doctors.
Below is my case information. Tried my best to convey briefly.

I am a male of age 37 Years with athletic body weighing 73 Kg. Height 5'-9'.
Masturbated a lot since the age of 13 years. in start 2 3 times a day and after the age of 20 almost 2 times in a week.
Married in 29th year of my age before 7 years.
Have a baby boy in 1st year of marriage which is of 6 Years now.
After the birth of baby boy i used condoms to get a gap between the next one. almost a year of safety we tried to get another baby. After 2 years of un succeeded tries, me and wife took tests. wife was fine according to gynecologist but my semen analysis shows low sperm count only 10/ml, low motility.
I started homeopathic medicine from a local homeo doctor. 02 Years before my wife get pregnant but unfortunately she bleeded a lot in the 3rd month and aborted. We were still trying to get our 2nd baby. I got tested many times but there was no improvement.
Currently my report shows sperm count only 02/ml, Motility nil, liquifaction time 30 Min, Concluded oligo & asthenospermia.
Feeling myself weak, low desire, erection problem.
A year before i took PTK 40 almost 4 months. it corrects the errection but no improvement in semen analysis. a month before i started

4 Drops each in 15 ml of water 2 times a day. getting desire, 80 Percent errection but no improvement in sperm count a motility after a month of usage.

I am a smoker of 15 Cig a day.
Strong body, Energetic, No constipation problem, having 2 Cups of tea daily.
I have a lot of mole like buds on my both testis. !st there was only 2 moles but now i have examined and found a lot littles. a lot of red veins can be seen on the testis. i can provide close pics. there is no pain in these. maybe it is because i use razor to clean the testis hairs.
