dr Kadwa please look into my issue of stomach and help me

1. Age,sex,weight,country,occupation.
ANS. Male 32,52-55,Pakistan,telecom engineer

2. Main complaints and other associated troubles.
1-Acidity and heart burn(Most of the time,after meal or empty stomach, even sometime there are cuts on my tounge and inner part of mouth)
2-Urine Issue (sometime pain, when start first one to two drops are looking thick and yellow. But it is most of the time. Sometime when i used much water urine will be fine and white. Sometime feel iching in my kidney but sometime, not every day)
3-clustro phobia(once i was gastric.acidity issue and i was felt suffocation in my room. At that time i was thinking to break the door and went out side. Still i am facing this issue. I avoid to set and live in closed places,I just new one window and it should be opened.Like now grill or iron net over there.)
4-hands and feet will be cold in winter and hot and burnt in summar even i dont like to put my hand on my legs or any part of body. It looks very hot.same condition with my feet.
5-small pimple on my back side,redish.
6-Contipation most of the time(very hard stool,i push outside but unable,i supposed i am unable to push feel pain in my bowl,much smell)


a)Where is the trouble; The exact locality of the complaint like hands,legs etc; duration of trouble.
ANS.when i have above situation i felt everything, pain in my bones,in belly,sometime headache,chest everywhere
b)What exactly do you feel, Sensation as pain, how pain feels or burn etc.
ANS. it like burning and sometime like Acuity in my stomach. I felt acid some of the time in my mouth and neck.Neck was burning. Its like vomiting was starting.i press and massage on my chest for reliefe.
c)What are the factors that causes this trouble according to you.
ANS. not sure, sometime i eat fast food,rosted food,pakoras etc but no heart burn i felf after eating.but most of the time if i just dring milk shake i felt burning.
d)Condition under which the complaint is reduced or you feel better like,cold or hot application,cold or hot weather,position as standing,walking,rest etc.
ANS. cold weather i like, in hot weather my symptom increased. i feel better after using cool water just for 10 to 20 minutes or sometime duration increased

ANS. in summar i felt i am upset beacause of above mentioned issues.

ANS.rare..dryness,color fanted,depression,sleeping disordered,thirst
g)Onset time of troubles in detail, i.e which came first, after that what problem and so on.
ANS. i analysis when i have constipation my other problem started. i felt burning in my heart,stomch burning,after that i feel there is issue in my urine. my all issues are inter-linked. otherwise i am fit,confident,energatic,everything.i also gain wait if i fit in my health.When i got mentioned issue ,my weight loss even i looked a sick person with slim body. its like just bone on my body.i eat everything but not feel any affect on my health.
h)Treatment method adopted and its result.
ANS. i started with allopathic but it was not permanent. used most of the madicines so i tried homeo from last two years i am not cured but little in better condition.

ANS. my father had stomach issue. then he had started heart problem in the age of 45.Now my mom has also acidity issue when she was tense and think on any matter deeply.

a)About childhood.
ANS. i was very sensitive in childhood. i studied in co education. So i loved girls. i started masterbation when i was 8th class.I think i was did frequently. but i am confused about frequently or not.But i am sure i used to do that manytimes.my mother said when i was child we fight each other with my brothers and sisters.most of the time my elder brother put a pillo on my face and ride on my face so i felf saffocation and cry. i was saw a bad dream in my childhood.It was something put on my face and i felt saffocation. then i wake up with crying and rush away.So this is some histroy.
b)Academic performance.
ANS. I am bsc in telecom
c)Any major incidents in life and the effect of it on life.
ANS. Death of my father. i am tired and feel bad beacause of the bad behaviour of people.
d)How you are satisfied with your sex life, friends, family members, company etc.
ANS. there are some issue in my family and i am distrubed, my sex life is good but sometime i released after 2 to 3 minutes.But i felt need of sex after one hour again then i take time to release.my friends are good and company is just some selected friend because i am outside of my home town.

5. Habits/Addiction.
a)Smoking, Alcohol,Sleeping pills, Laxative etc.
ANS. i take Pan(Chew tabacco with Pan leaf)
b)Masturbation and frequency.
ANS. rare..

6. How is your Appetite and Thirst.
ANS. i feel thirst and appetite is irregular sometime feel hunger and most of the time just full my belly to spend time.when i fit i feel hunger and appetite was very good and eat much.

7. Likes and Dislikes.

ANS.when i used sal much in my diet i felt pressure on my head and sickness same when i use sweet in access.

ANS. i like cleaness in my room as well as in my body. i like to walk and fresh breath,i like walk.i dislike extra pressure,abuse,and ill-mannor and ill-behaviour people.

8. Bowel movements.
a)Nature of stool, frequency, satisfactory or not.
ANS.hard and then soft.
b)Any discomforts associated with stool.
ANS. yes as i mentioned i got constipation

9. Urine.
a)Frequency, nature, volume.
ANS. frequency when drink much water otherwise its fine
b)Any discomfort before, during or after urination/odour
ANS. yes most of the time feel little pain in legs,and weakness

10. For men.
a)Any difference in erection/want of erection/weak erection/Ejaculation early/late.
ANS. sometime ejaculation early and little i was late. much erection when i saw nude women and when i was with my wife.i saw many blue movies but i was not satisfied because when i left to complete a movies i was unsatisfy.i supposed i am searching something i dont know what is it?
b)Any other trouble in sex.
ANS. no trouble

11. For Females.
a)Menses, Regular, Irregular,Early, Late.
ANS.

ANS.

12. Sleep.

whether the window must be open or closed etc.
common dreams, peculiar sounds or gestures during sleep, etc.
ANS.7 to 8 hours of sleep but rest less.but when i slep i dont know anything.position of sleep no idea.
i go for walk around 7pm to 8pm sometime after dinner and most of the time before the dinner.yes i need cover on my body during sleeping except my feet and hand in summar.i like open window.dreams are not particular i forget dreams most of the time.

13. Sweat
a)How much, what parts, staining, Odour.
ANS. much in sammar,underarms and whole the body,yes odour but small

14. Weather
a)Tolerance to heat and cold, dryness, humidity, weather changes, sun,
foggy weather, wind drafts, closed rooms, etc.
ANS. i dislike heat in sammar i try to avoid because i felt much heat in my body, cold i beared. But if i pick a block of ice it was unbearable after 1 minutes to pick and same situation if i pickup something heated. i dislike closed room as i mentioned above in my syptoms.

15. Mental Status
a)The quality of the patient's life in relationship to loved ones, family, friends and colleagues. Overall quality of energy available to function in daily life, and under various circumstances.
ANS. i am good in relationship i love my wife my kids and my friends as well.i tired if i Facilitate a sigle person among many people over there.i like to do equal and i like if they do good to me.
b)Any mental/emotional shocks occurring in the patient's life-grief, major financial losses separation from loved ones, death, identity crisis and other stress in life. no
ANS.
c)Memory,ability to concentrate/comprehend.
ANS. concentration in not good. in simple matter i try to consentration properly but if there is a difficult issue i do'nt concentrate on it like numbers and relations
example if someone say me i have a brother and he has two sisters and one son,one sister is married with son of my brother what is the relation between that girl and boy. so i totally confused.same if there are much calcutions and formulas i confused to calculte.

ANS. In darkness i felt saffocation

ANS. i mentioned detailed about clustrophobia and darkness issue

ANS. Try to calm down. but i felt i am impatient but i control my self.

ANS. yes i think i am emotional.i weep in alone,and keep silent.Sometime aggressive if a person abuse me or hit my self respect

ANS. poor to remember names,places,people but try to remember that i read.but the people or places and names that are associated with me i remembered them.

ANS. my facial of face is good when my health is good my color will be fair,my tongue is little red and when i feel acidity there are white surface and cuts on it.
