Need Help to gain Weight

I am 29 years old married and have a Kid of 6 years old. I am feeling very week and also have loose weight from past one year, due to which I feel very sleepy and unable to do my routine work. I am taking alopathic vitamin medicine but seems not to work. From past one year I am having a severe headache also I had many medicines to cure this headache but it still exists. Due to this weight loss my breast size has also decreased, before it was good looking but now it had become flat and skinny.I have loose so much weight that my skin is stick with my bones.My periods are also abnormal. Some time I get my periods twice in a month. Have acidity & gas problem also. Due to weakness I am having low back pain and feel very unhealthy to get continue my work. I please doctors to help me cure this weight loss problem to get my husbands attraction towards me again.

1) Since when you have been losing your weight?

2)Is there any mental stress or some issues that is taking toll on ur health? Please explain in detail

3) Have you been taking any medicines for loose motions?

1) Exactly i can say i am loosing weight since past 2 months.

2) i am not having any type of stress or any tention.

3) I am not taking any medicine for loose motion. I am having constipation, but have not taken any medicine for that.

4.Weight and height
5. Describe your family background, your educational qualification, and occupation
6. Describe your main and associated complaints in details. What things aggravate your complaints and things that ameliorate them?
7. Describe the events before the complaints started. The events can be related to mental or emotional state or it can be any physical event.
8.How exactly it started?
9.How do u feel about it? How does this complaint concerns you? And what comes to your mind when you think about it?
10.Have you taken any treatment or medication for it? List them.
11. Do you have any family history of similar complaints. List them.
12.Describe any life event that has made major impact on you. What are the issues in your life that's bothering you. Kindly describe the mental or emotional state in detail.
13.Describe you behavior, your mental state with your family members, and outside the family. (anger, irritability, anxiety, fears etc) Explain how do you react to these behaviors. If possible provide some examples to get clear idea of your mental state.
14) How do you react when you get angry?
15) Does empathy or sympathy affects you? How?
16) How do you overcome your mental stress?
17)Describe your desires or craving for food, and aversion to food.
18)How is your thirst? How frequently you drink water?
19) Describe your sleep, dreams, appetite and general energy level.
20) What kind of weather suits you the best or u like the most?
