4 year old attention problem

Hi
I have a son who is 4 almost 5 years old who gets distracted very easily - especially when it comes to using his cognitive skills. He doesn't want to do anything academic. Like writing his name, coloring, making lines or even scribble on a paper. I've tried every tactic to help him get interested in coloring and writing his name at least at his age but he doesn't seem interested and if he tries he gets distracted on things he shouldn't get distracted on. For example at school he is leaning to right his name and I practice with him at home. When I write a letter for him to trace he will just look at the letter and make comments and scribble on it but not write the letter on his own. When I read him a book he doesn't understand the story line when I ask him to describe what happened in the story I just read. He has a neurological evaluation coming up. Please help me what I should do. Even when I play sports with him he doesn't want to follow directions he Just wants to play with his cars but not throw the ball in a net.
What can I do to help him concentrated on his cognitive skills.

This is just a forum. Assume posts are not from medical professionals.
Please give him Baryta Carb 30 twice a day for 15 days.

One dose means
If the medicine is in pills form 2 pills. Don't touch pills with hand. Use cap of bottle to take pills.
If the medicine is in liquid dilution form, 2 drops in some 20 ml water. Sip up slowly.

Please follow homeo restrictions like no coffee, no raw onion/garlic, no strong perfumes, don't eat or drink anything within 30 minutes before or after taking medicine.
