Help with 6 month baby

Hello! Hoping to get some help here!

Trial of labor after C-section baby born at 40 weeks (30 hour labor failure to progress) by C-section. No complications of mother or child prior to birth.

Child passed meconium within 1 hour of birth and several times afterwards.

Child stooled infrequently after 1 week of birth.

Child completely stopped stooling after 4 weeks of birth.

Child only stools when stimulated by rectal exam or mother inserting pinky finger with and without a hollow catheter called a Windi. As of one week ago, child was given lactulose and solids were begun and child stools but painfully and seems forced with the medicine.

Child taken to Chiropractor and child stooled 1x per week when taken to Chiropractor and then after 3 weeks, sessions no longer worked.

Child nurses 7-8 times a day with good intake (both by bottle and by measuring feeds with a breastfeeding scale accurate to 2grams) On average child consumes between 28-35 oz per 24 hour period.

Lactation consultant suggested a mobility issue of the tongue and upper lip. Improper suction and suck seal causes excessive air which is thought to increase gas and reflux like symptoms as described by Aerophagia. Consulted with a dentist to perform a frenulectomy (Revision occurred at 8 weeks – mid February).

Additional lingual frenulectory performed by Ped Surgery team April 26th. Improvement noticed in latch but after 2 weeks, pain and gas/reflux symptoms returned worse.

Consulted with Dr. for a third revision of restricted tongue. Symptoms persist but have gotten relatively better.

GI recommended Barium Enema study. Abnormal findings of narrowing in the colon. Hirshsprung disease was a possible cause. Rectal suction biopsy revealed ganglion cells are present and Hirshsprung has been ruled out. GI performed several blood and stool lab tests which all showed normal findings except low Vitamin D. GI has ordered the following to help stool: Senna, Miralax, Lactulose, Milk of Magnesia, Black Strap Molasses, prunes, pears and extra water.

Child is breast fed.

Cystic Fibrosis was also considered. Newborn screening shows negative. No sweat test has been performed. GI ordered Cystic Fibrosis mutation blood work which came back negative.

Anal rectal manometry finds Anal Rectal incoordination of infant and has strong feeling it is partnered with delayed emptying of gastric chamber and “reflux”. Baby does not spit up. Was given Zantac for 5 days with no change. Prevacid for 5 days no change. Baby is currently on Zantac. There is some improvement but not significant enough.

Baby has been treated homeopathically for “reflux”. Had many remedies, I just can't remember them all but will recognize if mentioned.
Baby has also seen various holistic practitioners such as Cranial Sacral Therapy, NAET testing (food allergy), acupuncture and pressure.

Child chronically sounds congested at the nasal level although coughing indicates congestion in the lung area as well. Pulmonary described this as a reflux symptom.

Main concerns for child are:

Failure to gain weight at consistent rate

Failure to stool independently (in 5 months he has stooled on his own approximately 4-5 times and its usually wind passing that helps push out some stool) improvement with solids and lactulose but would like to take away the medicine lactulose.

Inability to sleep for more than 20 minutes at a time without being woken up by choking/gagging and what seems like holding his breath – there is also arching of the back and pushing away from whoever is holding him. If child is placed on his back he is immediately distressed. Child is put in a reclined position to sleep and after the 20 minutes have passed and he wakes, he still seems irritable and upset because he is tired. If mother is out running errands for other children, he will cry for excessive periods. If child is at home, 50% of the times mother is able to “rock baby back to sleep” and child will continue sleeping, albeit moving very frequently, for 30 min to 1 hour (this rarely happens).

Diagnosed Failure to thrive despite measured feedings at the breast and bottle. Reflux and colic like symptoms, painful nursing (Mother has noticed correlation of increased reflux activity if child is offered breastmilk too frequently or after comfort nursing and painful nursing from clamping down to stop flow because baby is not hungry yet continues to cry – mother cannot calm baby unless at the breast and that doesn’t work most of the time), baby does not sleep or settle.

Any other questions please ask.

I'm the mom. I am completely sleep deprived. Baby drains all my energy. I eat well. I eat all kinds of food. Not on any meds. I haven't done any blood work but feel ok..

Baby wakes up scared or aggressively after 20-30 minutes nap. Is still tired and upset. If I feed he acts very upset. Breathes heavily and needs to feel rocking all the time. Very high maintenance .

Arching and pushing is ALL THE TIME. Before feed, during feed, after feed, when trying to sleep wtc. Does not want to put down but also does not want to be held.

Foods - I like all kinds of food. I do crave a lot of dairy foods and wheat like breads. Deep fried is also very appealing. I don't hate food, I love it all. I lose weight very fast with breastfeeding . I feel when I have cut dairy from my diet, my milk supply drops.
My fingernails don't have a lot of white spots. Only one finger has a white spot.
Skin issues - baby has clear skin, I have clear skin. When baby nurses, milk comes out his nose. Either from not swallowing correctly or an over active milk let down.

He doesn't cry so much as complaining . Feels like he is trying to poop and pushing but can't come out . Poop was never solid before starting foods. It was always soft and like peanut butter. Sometimes it had a lot of air bubbles. After starting solids there is food in the poop. He sweats when he is nursing . Sleeps semi swaddled because he moves so much. Almost like a twitch. Has a lot of trouble sleeping.

You want me to take the Cina? What dose 30c, 200 etc? I also dissolve in water or can I dissolve under my tongue? I do not give the baby the remedy also?

Nataliesammi 7 years ago
30c and use water dose method mentioned above--only mum--babe will get thru breastmilk..

So I don't give baby the medicine to help him stool (lactulose ) or Zantac? When should I see an improvement or not, within a day or two?

Nataliesammi 7 years ago
meds very well may interfere with response --

after one night sleep should see some response--most like in child's emotion/behavior--that is key--if child seems more at ease after dose--good sign---from there watch and see how other aspects change or not...

if u feel meds being used are
life supportive and there will be a problem --then do not dose with cina ---and maintain course----someone else will come to help by prescribing while on meds--I am too much particular about this --from my learning

John Stanton 7 years ago
I understand. I don't believe they are life supportive and I wish to remove them from him so I will try your suggestion. I have just bought my remedy and will take . Will reply with observation. Thank you.

ok--keep eye for small changes ..new symptoms (never had before in ur or child health history)

John Stanton 7 years ago
So far no change. Although it does seem he woke up more often at night. Will advise later

Nataliesammi 7 years ago
ok--watch wait--if no response --reaccess from that view --

No new Or improved symptoms . Baby was a bit more miserable without his medicine to help stool. Today I had to stimulate him rectally to see if he would stool and he did but it was painful and hard stool. He will sleep 1hour or more on top of mother chest but can only sleep for 20/30 minutes if in crib or rocker. Someone please help us.

Nataliesammi 7 years ago
He still sounds very congested since April 1, has difficulty eating both nursing and solid foods. Always miserable attitude except after he stools then he feels good for a little time. Next time he eats he feels horrible again. I'm so sad that he is so miserable all the time.
