Please help me

Dear Dr
I am 39 years old male. hight 5 feet 9 inches. weight 70 kg.
I am very passionate and creative.Blood pressure is normal.
History- Masterbating almost 3-4 times a weak since age of 14-15 years till marriage at age of 27 years.
Likes and Dislikes: prefer sweets and sour foods.
less talkative. hasitating in public speaking.
Education: doctorate
profession: teaching
irritation on face from sun light in winters.
Problem: from last 5 years- pressure on heart. feeling of pain especially in evening and night hours below shoulder.
from last 3-4 months -Numbness in left arm and fingers of both hand (specially 2 and 3rd finger). and stiffness (finger not opened properly and remained jam) in my hand fingers. sometimes my heart beats increases in nights very badly. when i sit and take the deep breaths then heartbeats comedown to normal level. my mother has
