Dr Kadwa please help me.

I am a 49-year-old male, 5ft 5” and weight 80kg. I am a low immune person. I have been diagnosed with fibromyalgia, IBS-diarrhoea, chronic flatulence and depression/anxiety. I have chronic fatigue and sleep disorder. I am very sensitive to intense cold or heat. All this from last fifteen years.

Previously i have been treated with immuno-suppressants, Acetretin, cyclosporin, methotrexate, Prednisolone, Ultraviolet B rays. All my facial hairs turned white with UVB treatment. Recently i have stopped all the immune-suppressant drugs.

Lichen Planus: Thick, hard and pigmented skin on side of the sole of right feet. Thick skin on thumb and around nails with Black spots and hard skin on the toe and fingers of both feet.
On my right hand between thumb and first finger, the skin is cracked, hard crusty, thick and pigmented. They get dead white scales and if left alone will get cracked which pains a lot. I need to pluck it with nail cutter.
On my buttocks, it started with a painful boil and turns into crusty and itchy patch from long time. Which becomes raw and inflamed. Its same on my thighs like brown round spots becomes crusty and itchy. If plucked blood comes out.
Both sides of thumb are affected. And all the fingers around nail are peel around.
Seborrheic dermatitis in scalp, on the back of my head. Itchy and crust on top. Like boils on neck with a hard crust on top. Bleeds when plucked.
I go to toilet 3 to 4 times. Stomach is very sensitive. Saw some improvements in skin conditions on my back with probiotics and food supplements and vitamins mentioned below.
Sometimes at night and when I wake up in morning, I have irresistible itch In the scrotum and the area between scrotum and anal. I also get itch around anus. I keep picking the skin around the nails.
fibromyalgia pains: Pains in limbs and legs. Restless makes me struggle to sleep. Unable to sit long on chair or sofa. I mostly sit with legs high up on the table, or with folded legs. Back and neck pains makes me very irritable and stiff. Restricts my mobility. My feet’s gets tensed, tightness, immovable stiffness and numbness. I cannot touch the ground or let anyone touch my feet, it becomes very painful, immobilised and numb. Stiffness in different parts of body. It keeps moving. For example: Its one side of neck today then it’s another after few hours or next day.
Sometimes I need to be admitted to hospital with stiff back or neck. I cannot move and it starts all suddenly with spasms and later severe stiffness occurs. Make me Immovable with chronic pain and stiffness. I am given a course of Diazepam 5 mg followed by 2 mg for a week or two weeks. Pain killer naproxen. Earlier I had to take Tramadol. My lower back and upper back are both very sensitive. My shoulders and neck area are very sensitive.
I click my fingers of my hand and legs, repetitive all the time for relief. I have noticed pains in thumb, ankle, toe and fingers, where psoriasis/atopic dermatitis is present. I get pins and needles in hands and feet.
Sometime my vision gets blurred and find difficult reading on computer even with the correct prescription. I am sensitive to light. I get sinus pains and cold. I have a husky voice and could easily get throat irritation.
When I get, stiffness attack I cannot sleep. Normally also I cannot sleep at night. Nights, I get pains like flu symptoms. And whole body feels restless and painful. Although I am sleep I cannot sleep or struggle to lie down.
Depression/Anxiety/Panic attacks: I prefer my own company and cannot socialise with people other than my wife and son. In a second, anxiety changes my personality. I cannot take lies. I am a very simple living man. I cannot work with this pains and disability. I cannot wear shoes for more than two hours because of pains and lichen planus. I also use crutch on my right-hand side to walk. I barely go out. My wrist is very sensitive and sometimes find difficult holding the mouse or holding the mobile phone also. I am short fused person. Find difficulty to understand people’s speech. I get confused and anxious. I am very forgetful about medicines, forgets password even if I use it every day. Forgetful mind, short fused in a split second and calm down. Easily irritated and easily provoked by any one. stay away from people, crowd, and hardly go out and no social life.
Medicines:
Effexor Venlafaxine 37.5mg daily
Only when stiffness attack comes: Diazepam 5 mg followed by 2mg for one or two weeks.
Ketoconazole 2% shampoo.
Vitamin D, Magnesium, Zinc, Quercetin, Co-enzyme Q 10 Fish oil and Primrose oil.
