4 yr kids behaviour

Hi,
She is a very sharp girl but behaves on the extreme. Like she is very restless and talks continously and thinks continously something or the other. She cannot sit in place for a while.
secondly, sometimes she doesnt respect what we say to her. we request her hundred times say Hi to a visitor...she doesnt. Even you shout at her she doesnt obey you. At the same time when she sees the visitor some where she greets without a mention. she is aggressive when talking to friends in her class.But she fears her teacher.
when I say fear, becuase she was going to the loo often, teacher asked her to hold for sometime as others were also playing the same. Now she is not even asking her teacher though she needs to go to the toilet. she has urinated in the class twice...I am not sure what this problem is? Otherwise she is very sharp and grasps things very quickly. any help on this??
