2. Detailed information about the chief ailment.

' What is the diagnosis made by your local physician?

Ulcerative Colitis, Chronic Bowl Disease, Somatoform disorder, Menopausal symptoms, Anxiety neurosis, Nerve Root Compression. All these diagnosis during the period 1981 to till date are being over-ruled by another doctor. Uterus was removed in 1992 but uterus was quite clean & proper in size. Since the abdomen was opened, no physical abnormality was found. Exploratory Leprotamy conducted again in 1992 and no abnormality was found. But after Leprotamy, unconciousness and falling on ground or in stairs started along with problem of urination. Frequency in urination with pain. All the day and night I had been urinating and post-urination pain was felt. Urine started coming drop by drop. Ultimately due to frequent urination and pain, I used to fell unconscious. But this problem was over in the year 1999. All pathological tests shown no abnormality. RA factor is negative. Only sometimes, uric acid is indicated as 6.0 but that also varies. Last month, it was 3 and after 20 days it was 5. I mean Uric Acid remained within range. DNS(nose operated) conducted for headache but of no relief. Then patient was diagnosed as suffering from “somatoform disorder” but treatment and pscyotherapy (3 years) gave no results. Presently, patient is being treated for nerve root compression(MRI shown at C-4,5 and lumber region), osteoporoses, menopausal symptoms with pain killers for pain in all bones, nerves(not in joints) all over the body.

1. Abdomen pain (which lasted for 23 years) is not there presently.
2. Headache is mild
3. Acute pain in all bones and nerves, Stiffness in bones, especially cervical area, arms, shoulders, back of neck. Legs are also paining.
4. Arms paints extends to the tips of finders of hands and fingers are slightly folded inward and if try is made to streighten, much pain is felt. There is acute problem of KHARASH ( I don’t know the English word for it but can make you understand : skin irritation on the back and arms which if rubbed with nails, results into rashes. This problem is very acute these days.
5. Palms of hands are also paining and it is felt that as if these palms are burnt in fire. Pain is felt on palms of hands as if these were burnt in fire.
6. Sleeplessness
7. Low energy level that I am not able to attend to my homely jobs when I am totally free at home.
8. Smell. Petrol/Diesel/Cigarette/Biddi/Pan cannot be tolerated and cannot travel by bus or car due to petroleum problem.

' Please state since how long do you have your problem?

I have been suffering since 1981 with abdomen pain(pin pointing pain) along with headache. Abdomen pain continued for 23 years but now it is not felt. Headache is mild these days. Backache started in 2000 but it aggrevated along with pains in lumber area, cervical area, arms and legs after the patient met with an car accident resulting in nerve root compression at C-5,6 and L-4,5.

Present symptoms: 2005 : All bones and nerves all over the body along with kharash on the back and post-burnt feeling on the palms of hands and inner portion of arms. Hands and arms (inner portion of both) cannot be joined together as pain is felt like burnt hands joining. Those symptoms were relaxed (except nerve & bone pain) for two years. Now for the last 4 months, again these symptom appear and there is no relief. After 2 months kharash has also started which has become the most intolerable feature. Sleepless problem. Only 2-3 hours sleep is possible with sleeping pills.

May be due to psychiatric reasons
May be due to menopausal symptoms(uterus removed at the age of 35 but the overies were retained.
May be due to nerve root compression.
Kharash may be due to administration of some medicines.
I am sensitive to sulpha drugs like crocin, tetracycline, septran, analgin.
What are the factors which make your problem worse or better.

On movement, exertion, lifting loads etc worsens the problem. But without resorting to movement even pain in nerves, bones and kharash, pain in palms inner arms is so acute that intolerable. But most of the times, my symptoms get aggrevated in the evening say around 4.00 p.m. and remains till I sleep.

Sensitive to smell. Keeps everything clean. If not able to do myself, then mood becomes sad, over-perfection. Cannot travel in bus or car due to foul smell of petrol/diesel results in headache and uneasiness. In hilly areas, I remain relaxed of my symptoms. All the symptom aggravates in the evening say 4.00 p.m. to 11.00 p.m. in most of the time but these days kharash remains since morning and continues till I sleep. Energy level is very low. Cannot even attend to normal working at home like cleaning the house, cleaning of clothes. Even if I go to market for 1-2 hours on motorcycle in the evening, my energy is so exahused I cannot cook that night. Even my two young children comes weekly and I have anticipated anxiety that how I will pull on for their meals, etc.

Vegetarian. Cannot tolerate the smell of fish. Sour foot is also disliked. There is no problem with sweet and salty food. But to much sweet is disliked. Most of the times I like watery sabaji. Does not like Pan, elaichi, sofn, etc. smoking smell of ciggarratte of biddi is also not tolerable which results into headache and uneasiness.

i. More tolerant to heat No. Heat weather is not tolerable at all. I cannot withstand hot weather.

Now normal. After leprotamy, frequency of urine with pain as explained above.

I was in Govt. job as Clerk. Due to my problems of falling and becoming unconscious especially after Leprotamy in 1992, I left the job and now I am homely. Most of the time I am bed-ridden and cannot do much work at home.

My two sons are doing B.Tech and visit me after a span of 6-7 days.

Major highlights of your life since early childhood, if you think are relevant to your health issues.

I had problem with my mother-in-law in the year 1983. But now I am living with my husband and family since then. Now there is no such problem but if I remember those problems, I gets upset. I cannot say that has contributed to my problem or not as I have been suffering since 1981 much prior to the problem. After that we are living independently and spent about 24 years.

Yes I am over-sensitive and emotional. Gets angry soon. Thinks of future life. How I will be able to pull on my old aged life. Cannot make programme in a hurry. Cannot take decision in a hurry. I need time to make up my mind.

Due to disease only, I remain sad but it pains continues becomes depressed. I remain in angry mood but pains make me more angry. I do not have much fears except from water(pond or lake water). If people are sitting on the bank of a lake, a fear crept in my mind that they may not fall. I am fault finding in others lady. Most of the time, my feelings are negative. When there is no pain for a longer time, these symptoms improve slightly.

. DNS(Nose operated) in 1985 for headache. Uterus removed in 1992 for abdomen pain. Exploratory leprotamy done in 1992 for abdomen pain. Skin disease only kharash at the back and arms resulting in skin rashes. If my skin is hit a little, a black/blue mark appears and goes with some time latter. I am sensitive to Sulpha drugs, petroleum products, smoking smell, smell of incinerators, urine. Most of the pains are on left side.
