Sexual Problem:Urgent help needed

Hi Knowledgable souls ,

I am a male aged 24 years who used to masturbate on alternate days since I was 14 till 16.Then after that I started suffering from nocturnal emission(nightfall)for around 7 years. I consulted some local ayurvedic doctors but that was of no use.Then I was helpless and was struggling to stop this problem.Last year in April I started to suffer from another problem that is semen used to come after I urinated.Now daily after urination semen comes out and whenever I do attend the call of nature too.If there is a pressure then I can feel that something has ejaculated in my pants and this happens each and every day I go to the loo(when I pass my stool).I am suffering from severe mental pressure and depression due to this.Besides this,this has also led to pain in my ankles (more precisely pain in a vein in my ankle portion.not sure whether caused by this or something else).I can see a lot of veins on my penis and the erection is not stiff.Whenever I am distracted a sticky liquid comes out of my penis.I feel tired anytime and there is a loss in my grasping power.So, it is a humble request to the knowledgeble forum members who have expertise in this area here to please help me out and stop from my life getting ruined.

Medical disease history:
1) Was diagnosed with sinus last year.Followed the prescribed homeopathic medicine and since the last year there is no incident of sinus pain.
2) suffering from back pain.Was diagnosed with L5-S1 spondolysis last year.Pain is temporary depending on lifestyle.
