Loud Hissing in Ear

My grandfather is 78 and has a loud hissing in his left ear. According to other people and things he has read, he has TINNITUS. He has tried Niacin, but so far its not working. He says it sounds like a thousand insects buzzing in his ear, and it gets worse when he is relaxing, leaning back and going to bed. Can anyone suggest any remedies that might help?

Thanks!
