9years old son with learning and attention issues

Hello, My 9 year old son has the following issues: born 2 weeks before term by scheduled c-section. at 8 months pregnancy lungs were not developed enough so my doctor injected a substance to resolve the issue. during the last 3 months of pregnancy I couldn't eat any food without being sick, i actually lost weight. (he was my 2nd child, at 41 first is 10 ears older) my son was born with several cyst on both kidneys and yearly has to be motored but so far so good. he has been growing within the normal range, has a deficiency in Vitamin D
allergic to nuts, peas, lentils, has Asthma and Eczema. very early on in pre school he showed some signs of disobedience (wouldnt listen to teachers and follow rules) , would get upset if he didn't get his way. I then changed his school and he was super quiet for 6 months. then started in a different school ad again started to show the same signs. was an amazon reader by age 5 but didn't seem to understand what he was reading. also started very early on having learning issues: didn't seem interested in class, nonstop talking and disturbing class, had to have his way, would speak inappropriately, had difficulty making keeping friends. In the middle of 3rd grade I decided to homeschool him. His attention spam is very low, he needs to take break every 10 minutes. he is a very very loving child (constantly has to touch my face, and shows a lot of affection), is mostly interested in adults thins ( obsessed with construction work), notices and remembers details on people, very social , happy, repeats himself often, everyone who meets him say he is extremely advanced, but has a hard time following rules. no violence. but i am very concerned about his learning (when he applies himself he does really well ), he as a nice handwriting, great speller, he is now in 4th grade. no medication at the moment. can someone take my case please. we leave in the US.
