Erruptions on chin

I am getting eruptions on my chin and around the mouth area. It has been there since I was a teenage but it has worsened since last couple of years. I am 31 now.
The eruptions are red medium bumps forming white head after 1-3 days. The white discharge some time comes out or sometime dries off on its own. The skin then peels off and sometime it leaves red marks behind.

Any help will be greatly appreciated and please let me know if you need any further information.

Thanks for your reply. I just had a few questions.

Should I just take the medication for one day and then wait for the outcome. If so how many days should I wait.

I took the medicine as suggested and no major breakout since then.
Could you suggest something for scars and to smooth out the skin. I am also getting couple of small liver spots. Could you suggest something fot that too.
