Another case for Sameer, particularly about dosage...

Hi Sameer,

Let me run one more case by you. I have already given a remedy to this one, and I think this is an easy case to identify the remedy for, but I would appreciate your suggestions for dosing.

The case is a mother aged 63. At the age of 28, she had a miscarrage, and had a healthy child one year later.

Her husband has told me that in all of their years together they have only fought over one thing, money. The families finances have always been stable at some times a little better than at others, but she has always held down little ways of making extra money. She acts as though they are poor and believes that they are poor. She is very tight with money. She does not buy clothing, and very rarely will buy something that she wants. The husband feels insulted that she is always so scared that they are are poor when the reality is that they are not. She often has little side jobs or money making schemes. She is particularly nagging.

And she is never happy with whatever one in her family has achieved.

She exhibits a coldness to her children, and to her two sisters. She is not particularly warm with either of her sisters, and is often involved in family issues financially. She can be bitter towards those in her family of her age or younger. She is never giving and uses everything as a bargaining chip to get what she wants.

I think that she naturally is loving and motherly, but she does not express this at all. She is cold, harsh, and encourages a take no prisoners approach.

She is slightly overweight in the belly area and around the legs. She has black hair with a bit of grey. For years she has had varicose veins in her legs. She has had constant complains of back pain and has seen chiropractors for many years. She likes to have people crack her back, and says that helps her back.

She is very sensitive to any medicine, and she cannot even take a sip of wine or alcohol or she will pass out. She occasionally craves sweet things, such as cookies, cakes and pastries, but she will only eat a tiny bit of them.

She will often take naps during the day and will sometimes be tired at other times.

She has used many forms of natural medicine. Her mother died of lung cancer. She is partly vegetarian, but also has some meat at times.

This is just a forum. Assume posts are not from medical professionals.
She has had these symptoms regarding money for years, but they have gotten worse over the last year or so as she prepares for retirement.

Also, she is an intelligent woman, with a masters degree, but she has little ability to read. She will often read road signs out loud incorrectly, which is very embarrassing for an adult, and makes one look quite stupid, but she doesn't seem to care. As a child she had these reading problems very severely.

She will often say things that are totally socially awkward, and doesn't seem to notice or care.

Also, her constitution is primarily choleric, with a secondary Phlematic nature. She is most lacking in the aritable element.

Though she is otherwise quite sane, she has said that sometimes in private she talks with her deceased mother for advice.

I think I said general things about dosing in other threads (The mez. one today) and earlier as well.

Let me know the specific dosing situation in this case, and then I can tell you my opinion.

For this patient I gave Sepia. Because she is so hypersensitive, started with 30c. Again, I'm going to have to change my dosing strategy with all of these patients.

Dose 1:
30c (1 pellet dissolved in 8oz of water, from that one drop to a 8oz dilution glass) and gave her one drop of that. Immediately she took on a more cheerful tone and became more rational about money. This lasted for about 10 days, upon which she got more irrational about money. Her husband noticed the improvement, but she would not admit to noticing anything.


After 10 days, dose 2:
One succussion to the remedy bottle, one drop from that to the dilution bottle, and then had her take one drop of that. She then became a bit better for about 10 more days until the symptoms returned.

Finally she reported:
'Knees are better. Back is a little better.'

She described the following as a symptom, in her words:
'If you took your thumb and put it on heel and pressed it not very hard, it's like Ouch. Like a stabbing pain. Like pushing a nail into your heel. Has had before the remedy. Has not changed.'

Dose 3:
Two succussions to the remedy bottle, one drop from that to the dilution bottle, and then had her take one drop of that.

A few points which I noticed


1) With centesimals it is better to start with 6C for hypersensitive patients. Additionally, a 6c can bear repetition better than 30c for reasons stated elsewhere.
Also, it leaves room for going to 30c.

2)Two sucussions or even 10 will do nothing to change the dynamization at 30c, you are essentially repeating the same potency again and again. This is the reason why LMs were built, due to 'gradual yet perceptible (by vital force) enough' change in the dynamization possible through successive doses. However, LMs are not tolerated very well by some hypersensitive patients as they are extremely penetrating.

I think, if you repeat 30c for 3-4 more of these doses, she will lose all of her mental improvements as she is very close to fully responding to a 30c. You can try it if you want to learn it by doing :)

In my mind, there are 2 options:

a) Go to 200c, and give her atleast 1/4 teaspoon from the remedy bottle directly and wait for 3 weeks, and do not repeat 200c, as this should achieve whatever a 200c can in this case

b) Go to LM1, but this you should do if you are 99% sure about the remedy choice as the margin of error with LMs is very low.

Oh, one additional bit of information about this case, which the patient didn't tell me until after I had given her the first dose of Sepia, and she responded well.

She has a patch of dry skin on her face, which is different from the rest of her face. She had the dry area tested for cancer, but it was negative. She attributes this dry skin to age.

I'm not sure what miasm that would be or what that would have to do with.

I am really not sure I can go to 200c for this patient right away, as I've only given her drops of 30c.

What about moving to a 100c?

What about giving her 1 teaspoon of 30c directly from the remedy bottle?

This patient also seems to have some improvements at the psychological level. It probably would have been better to give her a 1/2 or whole teaspoon, rather than 1 or 2 drops. Reports that back and knees are still better. She doesn't attribute much to the remedy. Also no skin eruptions.

Hi Sameer,

You were right about this! Another dose of 30c, in this case with an increase to 1/2 teaspoon yielded little improvement. The patient was advised to take another dose of 30c with a full teaspoon. We will see if that brings any effect.

I'm sticking very closely with David Little's dosing techniques, and he is very big on increasing potency very slowly, while other homeopaths make Kentian sort of jumps in potency. David says that we need more potencies like the 60c and 100c that are steps between the 30c and 200c. He notes that Hahnemann used many more gradations of the C than are made to homeopathic pharmacies today.

I want to move this case to LM1 if possible to 'speed the cure' as David calls it.

Yes. If I think the patient can detect when to take the remedy I'd like to move to LMs. Otherwise, I will order a 100c.

David Little says that the first generation of homeopaths often referred to the 200c as 'the great aggravator', and the last thing this patient needs is aggravation.

I told her husband to give her a dose of 1 teaspoon (last dose was 1/2 teaspoon), but haven't heard if there was a response or not. It will take a week to get her the 100c or LM. If there was no response, I'd like to suggest a 2 teaspoon dose of the 30c. Would this cause a problem?

Also, I've discovered that I've been using too much water with my remedies. I've been using a 1 pint bottle for the remedy, and a 1 pint dilution glass.

Once again, you are repeating the same potency too much, and sooner or later, this will either start undoing the progress made or it will give rise to accessory symptoms.

A 200c is a fairly aggressive potency and the the body will not tolerate more than 2-3 doses of it, and that too when the repetition is done after at-least few 'weeks' only.

I would not recommend moving to LMs right now as the choice of similimum needs to be confirmed further.

I am going to follow your advice on dosing. I will try to acquire the 100c for her.

I do think that this is the correct remedy as her response to the initial dose in drops was excellent. For this patient, the most serious symptoms are irrational financial anxiety, that drives her husband crazy and alienates her from her sisters, children and others. Also, some hot flashes from metapause, and back problems that bother her. She has a Coleric temperment and black hair with some grey. Since metapause she has become more nagging, tempermental, never happy, never pleased, as though nothing is ever perfect. Her husband said that she was financially cheap and always thought she was poor when he met her, but that this has gotten worse, particularly in recent years.



Why are LMs more essential to use only if you know the remedy is correct compared to Cs? I would think that the C's would be more important to have the right remedy on.

Two years ago, a homeopath gave me the wrong remedy by LM. She gave me Rhus Tox, when I am a Calc-C, and it was at least better than 200c of the Nux Vomica she had given me before. It was gentler and there was at least some improvement.

Well the bad response to a remedy depends on the remedy choice, the closer the remedy is to being the similimum, the more damage it is capable of causing.

LMs are capable of causing the worst aggravations of all when used as the incorrect remedy or when used with incorrect dosage. I am sure if you try Nux Vomica LM1 (with a median dose) it would cause you worse aggravations than a 30c or even a 200c.

I would recommend going to 100c, and then moving to LMs if needed.

Follow up. I gave this patient Sepia 100c on Jan 7, 1/2 tspn in 1pt of water. The patient continues to improve slowly and gradually. Other than delusions of poverty, she is doing well in all regards.

I see some aspects of tb, the figwart miasm and psora and syph improving. If anything it is the general demeanor that is improving slowly.
