Size of tesicles

I want to invite opinion from all Homeo Drs yhay whether size of testicles has any impact on sperm count and motality??My patient has a bit smaller testies than normal size.

  mrbaig on 2009-07-23
This is just a forum. Assume posts are not from medical professionals.
Hey first of all how old is your patient, according to European standards, the following are the criteria for the testes size and position, contact me if you need any more clarifications, and i don know what you claim as normal, because usually the exact size of the testes can be measured only using an USG. read the following and see if you could get the size.

While the size of the testicle varies, it is estimated that 21.9% of men have their higher testicle being their left, while 27.3% of men have reported to have equally positioned testicles. This is due to differences in the vascular anatomical structure on the right and left sides.

In healthy European adult human males, average testicular volume is 18cm³ per testis, with normal size ranging from 12cm³ to 30cm³[3]. Measurement in the living adult is done in two basic ways:

* comparing the testicle with ellipsoids of known sizes (orchidometer).
* measuring the length, depth and width with a ruler, a pair of calipers or ultrasound imaging.

The volume is then calculated using the formula for the volume of an ellipsoid: 4/3 π × (length/2) × (width/2) × (depth/2).

If the size of the testicles are very small which means that it is undeveloped testes then it might affect the sperm count and motility. before we could make any direct comment first calculate the exact size and then we will see what could be done.
Thanks.

With due respect,
My understanding and readings of 14 years, interections with infertility specialists, gynaecs, sexologists and surgeons make me to differ from the other opinion.
Indians are smaller and leaner in size when it comes to westerns standards(especially European). Obviously they ought have smaller testicals which should be considered normal. However, across the indian parameters, if somebody has smaller testicles than its matter of investigation and research whether it co-relates with Sperm index(is calculated based on three parameters sperm counts, size, health and motility). However, if patient has god sperm count which is low than it can be inhanced with innocent remedy(nutrition) Co-enzyme Q10. If that reaches significant level than you can get the parenthood. If sperm count is zero than surgey may be option which may result/ may not result in some sperm count.

Dear Mr. Baig

Small testicles do not necessarily mean low sperm count.

A normal homeopathic case taking will lead you to the miamatic condition and the right remedy.

Patient is my son.Aged 34,married for last 4/5 years,no baby yet.In last three years tried all sorts of cure like Alopathy,Homeo pathy ,Ayurvadic etc but no success. Sperm count does not exceed from 10 million. Some times it becomes Zero.
Motility 20% and Morphology 30%.Ultrasound of Testies revealed :
Left Testis 2.7 X 1.4 cms
LEFT EPIDIDYMIS 0.7 X 0.6 CMS
Right testis 2.6 x 1.3 cms
Right epididymis 1.0 x 0.8 cms.
Radiologist remarked as both tesicles below normal size.

What do you say now??

mrbaig last decade
Well Mr.Baig.
Its interesting.
It has definately got cure in alopathy as patient has got some sperm count and it is not ZERO.
Our Homeopath friends will also help by their expertise.

Starsolyjoy last decade
If you don't mind, the sperm count became ZERO when ever tried Homeo remedy because Homeo Drs do not allow any vitamin supliment during their cure.Though I have full faith in Homeopathy but the truth should not be disguised.
I can give you the list of Homeo medicines given during this period. The Homeopath is MBBS too.

mrbaig last decade
ok we have a confused, complicated discussion, why don you give the list of homeo medicines your doctor gave you.
And vitamin supliments are allowed only in necessary situations. well give us more details of your son, like his behaviour etc or simply ask him to answer the following case record we will see what could be done.
Thanks.

Thanks for your reply,
you have been given with most possible medicines that could come for your case, out of which lycopodium and calc carb comes closer i prefer you son take the following medicine,
Silicea 30 one dose first day morning and the next dose the next day morning both empty stomach and just wait for 15 days and then report. see if he is showing any change in the sexual desire or feelings. lets see how it goes?
Thanks.

Sorry to interrupt, Dr. Bagyavasan. I believe, some break should be given before any new treatment be given. He has already taken an incredible cocktail of deep acting medicines.

My opinion would be to put him simply on Vitamin Supplements for a couple of months and get back to the case with complete details then.

Niel

Dear Neil,
Thanks for your opinion, i did think about it before giving silicea, I also appreciate the fact that he has been given a lot of deep acting remedies, but my plan of action was trying to understand him with the present totality, the presentation he is showing is with the effect of the all the medicines and his problem togather, so thought that if i could prescribe a remedy for the presenting totality(layer by layer treatment by George Vitholkas) then it should work even with the bad effects of the medicines given so far. so was the decision to give lower potency and not repeating it at all, i am planning to put him on just these two doses for the next 2 months and see how it goes?
I am also going on the same track as you 'waiting and watching' but i decided to throw the medicine that presents with all the factors and then to wait.
would be happy to hear your views on my plan.
Thanks.

Hi

I would still want the secondary action of the vital force to adjust to the primary action of the non-indicated remedy among the above list. (Aphorism 63-67 : Organon)

Vilthoulkas style treatment, to my understanding and in my opinion, would be most effective after we have given the Vital force some time to come to equilibrium.

If some plan has at all to be followed right now, it should be anti-doting if there are any proving symptoms from the above list manifest in the current situation.

That was my opinion. Please go ahead with what you think prudent.

Regards

Niel

Hey Neil,
I accept that the vital force will surely react to the medicines so to get to a result first we have to find out when was all these medicines given and what was the gap, so Mrbaig, kidnly clarify when was the last time you had given these medicines and if so what was the last medicine given and right now what medicines is he taking.
Thanks.

Dr. Bagyavasan.k last decade
Completely Agree.

Thank u all for taking keen intrest in my case.Actually I am very depresed and grived father looking my only son with out baby.
Ignatia 1 M was the last medicine given about one month ago before going for semen test.
But stop, let me tell u that all was not waste.Sexual desire is inreased, semen has become thick, liquification is improved, but alas sperm count is ZERO and no improvement in motality and morphalogy as well. Both are 20% only. How one can expect fruit in such condition.

Now a days he is on an Ayurvadic medicine which is said to be very effective in this case for last thousand years. The course has just been started. And you wont believe that this medicine is recommended by a senior homeopth.
Course is of 60 days span but advised to check semen after one month.
This is the case.

Hey Dr(s),first tell me that:
1. Is the size of testicles revealed in USG is alarming?
2.Why the count varies from time to time??
3.It is Oligospermia even then is it 'Testicular Atrophy'??
4.Does it need any sort of surgeory?
5.Is it cureable??

mrbaig last decade
I am waiting for your reply Dr.

mrbaig last decade
Dear mrbaig,
1.the size is not alarming.

2.the variation in the count is very unusual, either you should have completely reduced count or the count should keep on reducing, but this is variation needs further research.

3.all oligospermia need not have testicular atrophy, and many testicular atrophy can have normal sperm count as well.

4.no need of any surgery at this point.

5.according to me if the size change and the oligospermia is congenital then it cannot be reverted, but if it is accured then possibilities are there.

Dr. Bagyavasan.k last decade
What do you suggest then?Which sort of cure will be suitable,Alopathy or Homeo??

Further more, what sort of tests and researche is required??

mrbaig last decade
I would suggest you undergo a testicular biopsy to find out the state of the vesicles and the extent of spermatogenesis. I am not aware of the allopathic management for testicular atrophy, as far as i know with my general medicine knowledge the scope is limited, but it is worth trying homeo for a while in an organised, regulated and classical pattern then you might have an edge.
Thanks.

Dr. Bagyavasan.k last decade
Very rightly said by Dr.Bhagyavasan.
In alopathy also many angles.
Variation in sperm counts is quiet oftenly seen phenomenon these days.
As I told earlier, You may not do anything. Just take Co-enzyme Q10. Its considered alopathy but its under diet and nutrition category and very very safe-innocent. This noble prize winner product should be able to increase sperm index in three months time. Take path lab test after three months. If significant increment than you can proceed further.
Co-enzyme Q10 (100 mg) thrice a day.

Starsolyjoy last decade
OK.Thnk you very much.I will contact you later.

mrbaig last decade
are you MBBS Homeopath Beggy??

mrbaig last decade
How one can say that the size and Oligospermia is congenital or changed?

mrbaig last decade
Dear Mr.Baig. I am neither alopath nor homeopath by qualification. I have started studying both around 15 years ago. I interect appx 200 leading consultants in various segments(Like gynaec, infertility specialists, cardiologist and so on) every month, since last 10 years. I have good classical homeopaths as friends and I study on my on.
Oligospermia in 95% of cases not conginal but it catches up with life style, environmental changes, dressings, food habits and so on.
Testical size is usually conginital but it develops in propotion with the age. However if something is wrong anatomically than due to Indian or south asin culture it comes in to knowledge very late. Some of the issues related to size can be solved with surgeries but hardly significant work is being done in our country. Even all infertility specialists, give very reserved opinion when they talk about this option in private(They dont disclose to patients openly).
