This is just a forum. Assume posts are not from medical professionals.
That is an interesting belief - do you have a patient or a friend who has expressed this? That is a new one for me but I still get suprised by patients even after many years.

The important thing with any symptom, especially psychological or mental ones, is to explore it fully. Unfortunately you can't often just go to the repertory and find what the patient says.

What does it feel like? What is it like for someone to have two hearts. Get them to visualize, imagine, express - and then you want to connect it up to the rest of the case as well.

Is this concept of Duality represented in other parts of the case, is it somehow representative of the source/substance of the remedy - dual hearts exist in some animals such as the Oyster. Some animals have more hearts - Earthworms have 5 I believe.

Another possibility is that there is another belief under that one - two hearts represents strength, or it represents being an alien...or something.

Any properly taken case is all about exploring anything peculiar until you get to the bottom of it, where you are sure there is nothing else.
