Child with a distractive mind

I am curious to know from the panel of doctors and experts if there is any remedy suited to my son aged 11.
He is frail, looks underweight. He has a sharp mind, but looks very casual. The body language is not eliciting a positive response from people around him. e.g. He slupms, looks bored. Eats his meals very very slowly. As if he has no interesting eating whatsoever.He tries to keep up with a smart elder brother. He is intelligent and gets very good grades. He is not very obedient or maybe he's just in his dream world and not really listening to what is being said to him. Often he will ask to repeat the questions. He is of gentle nature, but is also naughty sometimes. Very curious about everything going on around him. Hence is unable to focus on the work at hand. Forgets things easily. Has no sense of time and has difficulty recalling mundane events like what he ate for dinner the day before.
I'm concerned about his health. Is there any medicine I could give him?

Some more characteristics.
His mouth is always open and he has formed a habit of breathing through it. A few years ago the doctor said he had an adenoid, but his symptoms seemed to improve so I didn't get it checked lately. He has braces on. Sometimes when I try to wake him up for school in the morning, I hear a grinding noise as if he's grinding teeth in his sleep.
Any advice will be greatly appreciated.
