pls help urinary problem

I am 58 years old and when i pass urine, the force is not there. it falls diagnolly. only if i have to hold urine for sometime, there is little force. at the end, urine falls slowly dribbling. also there is lot of Foam in the comode when urine falls there. i am very worried. history of excessive masturbation is also there.pls help.
