Thank you, anuj srivastava and rishimba.

Patient ID: Sex: Age: Nature of work: Habits:

Female, 40 years, Graduate student/Single parent to toddler

Please answer the following questions in a descriptive manner after careful analysis and recollection of previous experience and happenings.

1. Describe your main suffering? State the correct location.

During premenstrual and menstrual phases:
-Extreme irritability often taken out on loved ones.
-Annoyed by many things- noises, talking, being interrupted from talk or task, having people in my space. Feel aggravated by my toddler crying.
-Become frustrated very easily.
-Feeling very disconnected and becoming introverted.
-Having thoughts that I don’t express. When pushed to express feelings am unable to articulate well or really feel what is being asked of me.
-Feeling disconnected from self and from loved ones. Push loved ones away. Want to be left alone.
-An out of control feeling. I can see things are going badly (interpersonally) but don’t have the ability to see my part in it or change my behavior. Need space.
-Extreme fatigue and sleeplessness. Interrupted sleep makes me furious because I am so exhausted and sleeping so lightly.
-Extreme bloating of abdomen- look like I am pregnant.
-Extreme feeling of being uncomfortable physically. Nothing makes it better until my period starts or some days into it.
-Insatiably hungry.
-Very indecisive- far more than usual.
-Feeling very sad at times. This phase of the month is always accompanied by at least one bout of crying and feeling lost and hopeless- forgetting all of my successes and abilities.
-Accompanied by severe and ongoing headaches some months but not others.
-Always feel like I’m getting sick with a cold or flu that includes muscle aches, fog in the head feeling, and postnasal drip affecting throat a couple of days immediately before my period but those symptoms go away once the period begins.

These symptoms exist during the premenstrual and menstrual phases of the month only. Some months are worse than others. Some months are relatively ok. Symptoms are lessened with regular social activity and regular exercise such as running and biking, however, my lifestyle doesn’t always allow me to exercise and socialize regularly. Symptoms are also lessened when I get time to myself on a daily basis for meditation, yoga, or journaling- which never happens (for last three years) due to my circumstances.

2. What other physical sufferings do you have in your body?

I have lichen sclerosis of the vulva- a more mild case- considered an autoimmune disorder. It is treated with very small amounts of topical estrogen and testosterone creams. I was diagnosed with atypical lobular hyperplasia in one breast at one time but it is no longer visible in mammograms. I have nausea and significant bloating daily after eating- this started after my c-section three years ago.

3. What mental sufferings / feelings do you have associated with your physical sufferings?

I was initially devastated by the lichen sclerosis. It went 10 years without diagnosis or treatment because doctors did not recognize it and didn’t see the architectural changes that were taking place (even though I knew what it was and could see my body changing). I felt relief after a doctor finally agreed to take a biopsy for diagnosis and subsequent treatment- which has helped the condition significantly. I have come to terms with the condition and am not bothered by it unless it is in an acute phase. I just deal with it.

The breast diagnosis was also devastating initially. I have taken steps to maintain optimal breast health such as long term breastfeeding of my child and regular mammograms.

The daily nausea and bloating causes an uncomfortableness and is a pain to deal with every day. The bloating causes me to not like the look of my body during those times.

4. What exactly do you feel when you are at your worst? Describe the sensation in your own words.

I feel extreme irritability, aggravation, disconnect, out of control feeling, extreme exhaustion, fatigue, and physical uncomfortableness when I’m at my worst. Like I am in a tunnel and see no end.

5. When did it all start? Can you connect it to any past event or disease?

I have suffered PMDD symptoms for many years. 15 or more years perhaps.

6. Which time of the day you are worst?
The PMDD symptoms last all day and night during premenstrual and menstrual phases but are probably most heightened upon waking and when trying to go to bed at night.

7. What are the things that aggravate your suffering and those that ameliorate the same? Example: time, temperature, pressure, rubbing, washing, eating, tight clothing etc.

PMDD symptoms are aggravated by lack of alone time and lack of understanding by loved ones. Tight clothes or feeling of being trapped by circumstances make symptoms worse. Noise, talking, and argumentativeness by others make symptoms worse. Being forced to talk or being held accountable for the out of control feeling and disconnect I have make it worse. I am unable to be reasonable or accountable while PMDD symptoms are in effect. Symptoms are ameliorated by an understanding by loved ones and by them giving me space and time alone when needed. Peaceful rest, naps, and quiet helps alleviate symptoms.

8. Do you think your sufferings have relation to any external stimuli (like, change of place) or any internal biological changes in the body, like, menses (in females)?

Sufferings happen half of the month- during premenstrual and menstrual phases.

9. When do you feel better, during hot weather or cold weather, humid or dry weather?

I must have warm weather. 80 degrees with some humidity is perfect. I must have daily sunshine and lush, green surroundings. I must be near the ocean. Cold, dry, grayness, no sunshine, and leafless trees are intolerable.

10. Describe your general mental set up? Are you Moody, Arrogant, Mild, Agreeable Changeable, Nervous, Suspicious, Easily offended, Quiet, Arguing, Irritating, Lazy etc.

I am generally positive, happy, hopeful, loving, encouraging to others, very intuitive, adventurous, and social with loved ones and strangers. I make friends easily and am typically viewed as strong, self-aware, very capable, resourceful, and smart. During my premenstrual and menstrual phases (which are approximately half of the month) I am irritable, moody, introverted, resentful, angry- as described in my answer to question one.

- How do you feel before or during a thunderstorm?

I enjoy thunderstorms. I like the smell and sky before them and the sounds and rain during them. However, I become fatigued and depressed if it is gloomy outside for more than a day or two at a time. I must have sunshine every day.

- Do you like being consoled during your tough times?

Sometimes, yes. With positivity. It helps to have my successes and strengths pointed out as I don’t give myself credit for them when I am down. But sometimes I completely withdrawal into myself and don’t want to talk to anyone.

- Are you sensitive to external stimuli like smell, noise, light etc?

I am extremely sensitive to smell. It’s like a super power. I smell things before anyone else does and often when they don’t at all. I can smell things in the wind- like cigarette smoke or cooking meats- even from far distances. I become nauseated by unpleasant odors (including cigarette and cigar smoke, cooking meats, and smells in public toilets). I am sensitive to noises such as many people talking at once or TV or radio too loud. I am sensitive to fluorescent light- makes me feel foggy and sometimes gives me headaches. I am very sensitive to dusk- become depressed and lonely if indoors at this time and must turn on artificial lights and the TV. Am much better during dusk if I am outside- can even be enjoyable if on the beach or somewhere I like to be- like Disney World (I don’t mind the sounds and lights of Disney World). Effects are worse during premenstrual and menstrual phases.

- Do you have any typical habit or gesture like nail biting, causeless
Weeping, talking to one self etc?

Always feeling my front teeth with my tongue- drives me crazy.

- How do you feel about your friends, family, your children and especially your husband / wife?

I have a handful of very close and good friends whom I respect and admire. I have fragile relationships with both of my parents. I have no relationship with my sibling. I love and adore my child. I sacrifice everything for my child and make all decisions based on what is best for her. She is my number one priority and I strive to maintain a feeling of connectedness with her. I have no spouse. I have a significant other and the relationship is currently rocky. I care deeply for him but have some issues with the way he deals with my PMDD symptoms. He leaves the relationship completely which throws me into primal feelings of abandonment.

11. What are your fears and do you dream of any situation repeatedly?

I have a fear of my child’s completely and always absentee, unsupportive, abusive, biological father trying to get custody rights to my child at some point.

12. What do you crave in food items and what are your aversions?

I crave and drink green tea and/or coffee daily. I also crave and eat dark chocolate daily. I crave hummus, olives, vegan cheese, and Thai food. I crave salty and sweet foods during premenstrual phase.

13. How is your thirst: Less, Normal or Excessive?

Anywhere from normal to excessive. I only drink water (aside from green tea or coffee upon waking).

14. How is your hunger: Less, Normal or Excessive?

My hunger is excessive during the premenstrual phase of the month.

15. Is there any kind of food which your body can’t stand?

I have eliminated wheat and turmeric as they were making me vomit.

16. Is your sweat normal or less or more? Where does it sweat more: Head, Trunk or Limbs?

I’m not very sweaty. When I exercise it is probably more head and trunk.

17. How is your bowel movement and stool type?

Bowel movements three times a day, easily. Stool rarely hardened. Rarely constipated. I am in and out of the toilet quickly.

18. How well do you sleep? Do you have a particular posture of sleeping?

I sleep on my sides and back. I change positions often. I typically do not sleep well.

19. Do you think you are able to satisfy your sexual desires in general?

Yes. Very much so when I have a partner.

20. Do you have any strange, peculiar or unusual symptom or feelings? How are you different from others?

I think I am probably more in tune with myself (when not premenstrual) and intuitive than most. My sense of smell is very powerful. I am not very mainstream in my thinking. I am more concerned with health, travel, and being with my child than I am with expensive cars, clothing, etc. I am more interested in experience than money- although I don’t want to struggle financially.

21. What medications have been taken earlier by you to treat the diseases and do you have any particular symptom surfacing after the medication?

None other than half pea sized amounts of topical estrogen and testosterone a few times a week for lichen sclerosis.

22. What major diseases are running in your family?

Prostate can_er- paternal. Depression- maternal. Severe alcoholism- sibling.

23. Describe, how do you look like? Describe your overall appearance.

5’3” 105 lbs. Thin but athletic.

(For Females)
24. If your menstrual cycles are not normal, please describe the irregularities, like pains, moods, flow type, clots etc.

Cycle is generally regular. Had two periods in one month (including two horrible premenstrual periods) a few months back. Menstruation always comes with either new moon or full moon. Always know when I’m ovulating.

25. What major diseases have you had in your life and when. Please write them in a chronological manner.

Lichen sclerosis.

hi rishimba,

would you like to take over the case?

anuj srivastava last decade
Thanks Anuj for asking!

On reading her post it seems to me its a clear case of SEPIA OFFICINALIS.

Would like your opinion as well.

Best would be to start a low potency sustained dose. SEPIA 12C thrice a day for two days initially would be right. I tell this because she has some skin related problems as well. Its always right to start very low when skin is involved.

Birdwords, please stop using the cream that you were using for your skin eruption when you start taking the remedy.

Once you see the response, the second spell can be taken.

Lachesis would be my second prescription if sepia doesn't work.

Thank you so much, Rishimba. I purchased the remedy today and will start it tomorrow. Can you please tell me how many pellets to take each of the three times? And do I take it everyday? Or just for two days? Then what?

Thank you, again.

Birdwords,

You will understand that SEPIA 12C is a low potency. As per your post you have not taken any homeopathic remedies earlier. Moreover, you have skin eruptions. I don't want any flare up of the skin because of the remedy or else you will lose faith on homeopathy.

Anuj is spot on in his prescription. He prescribed for you Lachesis which was my second prescription. You should now know that these two remedies will cure you of all your problems if taken at the right potencies and right time.

We may have to eventually go for higher potencies based on your response but for the time being, lets start with SEPIA 12C and see if your litchen sclerosis can be cleared.

If you have purchased pills, take 3 pills under the tongue and let the pills dissolve by themselves and get absorbed. Do not take any food or drinks one hour before or after the doses. This is called one dose.

Take the doses, three times a day in similar manner for some days ( continue for a week maximum) as I do not know when you will get a response. In case you feel the skin has started itching or getting better, you should taper down the doses to twice a day or even once a day.

If you don't feel any response even in a week, let me know so that I can suggest a higher potency to you.

If SEPIA doesn't produce a response even in the highest of potencies, we will go for Lachesis. We will change remedies only when we are sure sepia will work no more.

It will be a relatively long and well controlled treatment. Please don't get impatient.

Thank Anuj for his hard work as well because he e-mailed me his recommendation.

Thank you, Rishimba. I would have also thanked Anuj had I known he responded. Thank you, Anuj.

I have been taking the remedy as you prescribed for three days now. I got a response the first time I took it. I felt very anxious and as if I were in a fog. The effects subsided after about an hour. Each time I take it I feel somewhat anxious but less so than the first time. The first two days I took 3 doses. Today I only took 2 doses. I'm under the impression I am not to take as many doses once I get a response? Will you please advise?

Thank you very much.

birdwords last decade
By 'Response', I meant the reaction of your body especially with respect to your symptoms for which treatment is being sought.

In a few days, you may come to know if your affected skin responded to sepia.

Take the doses for a few days more, maybe another two to three days maximum. If you do not see any change in the sensations of the affected area of your skin, stop the remedy.

After waiting for some days, come back here to share your observations. We may have to suggest a higher potency to check sepia out fully.

Ok. I will keep taking it. I am most concerned, however, with my PMDD symptoms. They are affecting my life in profound ways (causing severe problems in my relationships). It is truly the mental issues I suffer from the PMDD that is my priority to alleviate at this point. That is not to say I am not concerned with healing/curing my lichen sclerosis, of course. I am just very much in need of treating my PMDD.

birdwords last decade
I understand your concern. The reason I didn't suggest Lachesis because initially I wanted to start with Sepia in a low potency fearing your skin might flare up.

Listing your symptoms I came up with SEPIA and Lachesis. Because lachesis is inimical to sepia and antidotes it, I thought it better to check out with sepia first and then after an interval go for higher potency of lachesis.

Please don't get impatient. You will be alright.
