Ok no problem. Let me post my questionnaire. It is a standard one - feel free to digress from it if you need to talk about something that is not covered.

CASE PRESENTATION FOR CLASSICAL HOMOEOPATHIC PRESCRIBING

General Guidelines:

1. Try to be as descriptive as possible. When describing pains or sensations use descriptions along the lines of ‘feels as if someone is squeezing it with their hand’ or ‘it is like an insect crawling around inside’ or ‘it is as if someone is standing over me threatening to kill me’. Long descriptions are always better than short ones. One word answers are difficult to use successfully.

2. Another important part of symptom description for homoeopathy are the ‘modalities’. These are situations, events, activities or conditions which modify the symptom in some way. We usually refer to this as what makes them ‘better’ or ‘worse’ (amelioration/aggravation). The situation may actually provoke the symptom into appearing, make it vanish altogether, or just increase or decrease it in some way. Modalities are vital information for prescribing.

Modalities are typically related to (but not exclusively) the following situations:

A. Temperature, weather
B. Time (day, night, specific times, frequency, periodicity, season)
C. Position, activity
D. Emotions, thoughts
E. Food and drink
F. Drugs, medication

3. How the symptom is perceived is important too – odour, appearance, sound, touch. What colour is it, what is its texture? For gestures, describe what it looks like the patient is doing, or what you feel you are doing while making the movement eg. ‘they look like they are swatting flies’ or ‘I feel like something is pushing my hand upwards’ or ‘it is like I stick a fork in an electrical outlet which throws my body backwards’. Description – it is key to accurate prescriptions.

4. When did the symptom or set of symptoms start? The apparent cause can be useful in determining the remedy, although it is not of the same importance as the previous factors. It may have been a specific event, a disease, an accident or even an emotional experience.

5. Use your own words. Do not copy phrases or descriptions found in our various remedy pictures. Try not to use other people’s ideas or thoughts or words. If you are reporting on behalf of someone else, report their exact words, however you can also report your own observations of them (not opinions).

6. Does the symptom occur alongside another specific symptom? Do particular symptoms only occur together? Does a particular symptom occur with a particular thought or emotion? For example, a headache that always comes with visual disturbances, or stomach pain that appears alongside anger, or anxiety that makes you feel like running down the street screaming.

7. Each complaint should be described fully before going on to describe another complaint. Try not to mix different symptoms or complaints together. Each modifying feature must be clearly attached to a particular symptom/set of symptoms. Any mistake you make here is a mistake the homoeopath will also then make.

MENTAL AND EMOTIONAL DESCRIPTION



(Please spend the most time on this section and give DETAILED answers - the homoeopathic remedy is very often decided on the basis of the mental and emotional state of the patient !)


1. What are the issues in your life that bother you the most. Not physical issues but mental or emotional ones. List each one separately and describe why each one bothers you so much.

2. What emotions are the most troublesome for you? What situations provoke these emotions. How do these emotions make you act? Do you feel any ill effects from expressing or not expressing these emotions.

3. What incidents in your life have had a deep impact on you? Describe each incident in detail and how they made you feel? What did you do in those situations? What effect have they had on your life?

4. What are you afraid of? Especially important are phobias, but it might be objects, situations or events that just produce a high level of anxiety. How do you manage your fears? How do you react when confronted with these fears? What would be the worst situation for you to be put in that would provoke these fears? You may need to talk about each fear/anxiety separately.

5. What hobbies do you have? Why do you like each of these activities?

6. Do you have any persistent thoughts, ideas or beliefs that are difficult to stop or cope with? What are they?

7. Do you have any unusual gestures or movements of the body? Do you feel any unusual sensation or pain throughout your body? What exactly does it feel like is happening in your body?

8. When you experience your fears, persistent thoughts, or difficult emotions, what kind of sensation or reactions do you get in your body?

9. When did you feel at your best in your life? What was that like for you? If you imagine the complete opposite of this feeling or moment, what would that be like?

10. Do you feel like you are stuck in a pattern of behavior, especially when trying to deal with your problems? What is this pattern? THIS IS AN IMPORTANT QUESTION PLEASE CONSIDER CAREFULLY AND GIVE DETAILS.

11. What difficulties or problems do you have in relationships? Talk about your family, your romantic relationships, your spouse or partner, your friends, and your work colleagues. You may need to talk about all of these separately.

12. List 5 positive things about yourself. Are there any situations where this positive attribute becomes negative (is a problem)?

13. List 5 negative things about yourself. Are there any situations where this negative attribute becomes positive (is useful)?

14. Do you have any reoccurring dreams? Describe them in detail, including any feelings that come while dreaming. Dreams are very important in unlocking the deepest truth of a patient’s case, but it is not enough to simply describe them in a sentence. Give as much information as you feel comfortable doing.

15. Did you have any reoccurring dreams as a child, or earlier in your life? Describe those in detail including any feelings that came with them.

16. What were you like as a child, your character, your personality, your fears, your dreams, your problems?

17. What kind of environment did you grow up in? What problems where there at home, with your family, with your parents, with your siblings, with school?

- What foods do you hate eating (have an aversion to)?
- What foods have a negative effect on you or cause symptoms?
- What foods have a positive effect on you or seem to improve your health or symptoms in some way?

- What is the effect of hunger or fasting on you?

3. Thirst - What drinks do you crave/desire strongly?
- What drinks do you hate to take (are averse to)?
- When are you most thirsty?
- When are you least thirsty?

4. Stool - Do you have any problems with your bowels or passing stool?
- What is the shape, color, odor of the stool?

5. Urine - Do you have any trouble passing or retaining urine?
- What is the color, odor of the urine?
- Do you have any sediment or debris in the urine?

6. Sweat - How do you feel about the amount of perspiration you have?
- Where do you have the most sweat?
- What is the odor?
- What color does it stain clothing?
- Does anything in particular cause you to sweat abnormally?

7. Sexuality - Any problems with your sexual desire?
- Any problems with your sexual ability or function?
- Any history of sexually transmitted diseases?

8. Menses (Women)
- How many days is your cycle?
- How many days does the flow go for?
- What is the appearance of the flow?
- What is the odor of the flow?
- What kind of stain does the flow leave?
- Any discharge before, during or after?
- Any pain before, during or after the flow?
- What symptoms come before the flow?
- What symptoms come after the flow?

9. Environment – How does the weather affect you?
- How does the temperature affect you?
- How does the season affect you?
- What physical activities affect you?
- Is there anything else in the environment you are sensitive to?

10. Anything else you feel is important that hasn’t been covered by previous questions?

Glad to get your answer!

The problem with me and symptoms is that really I lack almost any physical ones and the mentals are also not a lot.

The main problems starts at an early age (13-14 y.old) when I started to feel a kind of disconfort in high school regarding jokes made by my mates in wich I felt afraid and suspicous with no reason really.

After those years I started to feel a deep sense of FEAR...at the beginning this fear was about getting into a fight with anyone that stared looking at me in a sense that i perceived the looks of others as a provocation....this lasts situations Started when I began College.As a result of all this, the first reaction was to confine myself at home, but after a while the symptoms appear anywhere, anytime (20-21 y.old)

Now I am 40 and even tought i am polimedicated the symptoms stays the same....there is (except for klonopin at the beginning) that works for my suspicion and constant fears.

Its worth saying that I had a difficult childhood, my mother after deliver me , enters into a post.puerperal mania in which she didnt let anyone to touch me, feed me, looking at me...even my father.-This situation subsides after 2 years, when my brother have born.

Also, I grow very dependant on my father who died of lung c. when i was 10.
I think I have never did a proper mourning, but my therapist now says that I keeped a lot of anger at him, that i dont really can recognize.

Up today I live frightened at EVERY look I cross, fearing that I could get into a fight and get hurt, but at the same time a deep anger grows inside me for avoid what i perceive as a provocation as I write before.

I am hipothiroid (replaced with levotiroxin) and also I was prescribed metformin for my overweight.

In my homeopathic history, the only positive hint was a dose of Stram XM that had a good effect for about 3 months and after that was tried to go up and down with stram and changing to LMs but nothings more happens.

Worth remark that after the benefit of stram. my symptoms remain the same, i mean, nothing changed.

Iam now totally depend on the help pf my mother...my 2 brothers live abroad.

I have gone trought 6 psyquiatrics admissions where the main symptoms where delusions of grandeur and paranoic feelings with great suspiciosness....like my usual paranoid symptoms becamed straight paranoia.

Other fears: dead, also great aids "terror"...Even when I never engage in any known means of aids transmission and that I can rational understand, I found myself believing that i got it because a spite of saliva enters my eyes or sharing a coke.Also i am very afraid of any type of aggresion and poverty.

I have a deep fear in social meetings; I have a couple of friends that i see from time to time, but social phobia is there present.

In the physical plane, there is the metabolic syndrome (insulin resistence, high colesterol and overweight) and lately the presence of boils in the perianal zone.

I use to eat whatever, I meant I have not marked dislikes when it comes to food.-Only that I love crbohidrates, pasta and yunk food.

I cant stand hot humid weather, it just change my mind making me worst.I love winter time.

When I get a deeper fear there is a sensation of drowning at the chest zone and anguish.

I need to write the list of homeopathic remedies that I have already tried and I would like your guide in the writing of my symtoms as well.

1. Sleep - what position do you tend to sleep in?

I sleep on my sides, dont have a constant one.

- what position can you not sleep in? proned

- do you do anything unusual in your sleep? I use to snore
- any problems with going to sleep, staying asleep, or waking up?
I have a really bad time getting to sleep, the sedatives I was prescribed induce only a light sleep.

2. Appetite - What foods do you crave/desire strongly? Italian.

- What foods do you hate eating (have an aversion to)? None

- What foods have a negative effect on you or cause symptoms? None.

- What foods have a positive effect on you or seem to improve your health or symptoms in some way? Sugar, chocolate

- What is the effect of hunger or fasting on you?

None.
3. Thirst - What drinks do you crave/desire strongly? I dont use to drink more than 2 glasses of liquid everyday.

- What drinks do you hate to take (are averse to)? High alcoholic beverages.



- When are you most thirsty? Never.

- When are you least thirsty? Always

4. Stool - Do you have any problems with your bowels or passing stool?Regular stools.


- What is the shape, color, odor of the stool? Brown, sulphuric odour and spinach like.

5. Urine - Do you have any trouble passing or retaining urine? None so far.
- What is the color, odor of the urine? Yellow, almost odourless.

- Do you have any sediment or debris in the urine?
No.

6. Sweat - How do you feel about the amount of perspiration you have? I perspire a lot...all of my body.I sweat with apple vinegar odour in my belly.

- Where do you have the most sweat? all over my body, especially my head.

- What is the odor? Except for the belly odour, the rest of my body is just as normal.

- What color does it stain clothing? None,

- Does anything in particular cause you to sweat abnormally? Feelings of fear or in social reunions.



7. Sexuality - Any problems with your sexual desire? None.The desire stays, but last sexual activity was 15 y. ago.




- Any problems with your sexual ability or function?
None.

- Any history of sexually transmitted diseases?
None.
8. Menses (Women)
- How many days is your cycle?
- How many days does the flow go for?
- What is the appearance of the flow?
- What is the odor of the flow?

Alright, let's explore a couple of the issues you have brought up.

Can you expand on the following statements or symptoms? Try to explain the feelings, situation, or issue in as much detail as possible, giving examples if you can. What I am trying to understand is what it is like for you during the feeling, event, or situation - what you think, what you perceive, what you do, what you try not to do etc. When you meant something you are afraid of, tell what that thing means to you, what will happen to you, be precise and descriptive.

Just write whatever comes to mind, try not to edit. If you have to correct yourself (apart from spelling) leave what you have written and correct it after in a new sentence.

Fear of getting into a fight
Provocation
Being stared at
Constant fears
Fear of being hurt
Deep anger
Totally dependent
Delusions of grandeur
Paranoia, suspicious
Fear of AIDS
Fear of poverty

Ok!

Fear of getting into a fight....the sensation is as if I will end up very hurt and if i do i cannot fight back...I recall that I never get into a real fight and even with my two brothers never...I need to prove something, that i have honor that I am not able to reject the fight as i always feel, but the fact is that cowardice is a big issue along with lack of selfconfidence in me.

Provocation: I huge issue because I am not able to read "the faces" that i cross in the street...even when i got a look of any men and the persons has passed before my eyes, I keep the resentment, deep guilt because i didnt do nothing and the fact is that I take my eyes apart but the provocation is felt right in my inspiration like i said "drowning" at the height of my chest.

Being Stared at:That makes me feel paralized in fear or may be i should say, in terror.But this feeling also appears in social meetings as well, when there is no place to hide my frozen expression of fear.

Constant Fears.I usually live "in fear", no triggers, no big motives, but since 26-27 y.old.this feeling stays with me no matter what.where or with who.

Fear of Being Hurt: As I learned in therapy there must be a lot of agression inside of me that i am used to project in peoples eyes and in fact at the beginnig of my symptoms (18-19 y.old) The fear and the anger (deep) were as one, i mean i couldnt diff. one or the other because were happens at the same time.

Deep Anger:This feeling growed stronger as my fears becomed more present, because as I said before I use anger ans resentment as synonyms.

Totally Dependent:As my life started to being ruled by these strong emotions I was not able to work and to earn my money and becomed dependent on my mothers.For food, house, psyquiatric attention and so on.

Delusion of grandeur: This delusions appeared only before admissions and general had it triggers...being them strongs antidepressants, abrupt discontinuation of medicines.-I felt like a great men, full of knowledge and having a great heritage, even one time appeared the idea of being the reincarnation of a great magician or worst the sucesor of the 3erd reich.!

Paranoia and suspicion, I would say paranoid, because even tought I am comvinced of the provocations in the streets, then I manage to rationalize that just It couldnt be that way....doesnt mean the feeling vanished at all.Suspicion for my deep mistrust in people in general.

Fear of Aids.This one has an especial place in my mind, I was diagnosed for it as OCD because of the fears are repetitive and clouded my mind to the point that i am get to the point of (again) frozen in/or with fear thinking about I must get the virus.So far I got my blood checked 9-10 times for it.

Fear of poverty: Sometimes I feel like "what will I do when my mother died" and then this feeling appears.

Social Phobia:This is because I find imposible to interact or stay arround people I dont know...I am not afraid of crowds, I feel awful when I met new people and it spects that i share with them.

Agression: The real agression appeared in my dreams...usually I dream that i am in violent fight where my oponent get destroyed almost to nothing.
Also fight with friend and my brothers.

Drowning:Its the caracteristical symtomp I use to feel during a fear aggravation.

Alright we need to go carefully here, as there is a lot of mental pathology and a lot of suppression by psychiatric medication as well.

It is possible for you to get Mercurius LM1 ( may be denoted as 0/1)? If not, obtain Mercurius 30c. If you are able to get both, do that. Obtain them preferably in liquid form.

If you instead have pillules or pellets, you will need a small bottle and a dropper to make your own liquid dose. Mix water and alcohol into this small bottle to the ratio of 5:1. Dissolve 3 pillules/pellets into this bottle. All doses will be made from this bottle.

If you already have a liquid dose just start from step 1.

1. Hit the bottle 5 times firmly against the palm of the hand

2. Place 3 drops into 100mls of clean fresh water

3. Stir very thoroughly

4. Take 2 teaspoons out into the mouth and hold for 20 seconds, then swallow.

This is one dose and the same steps should be taken for any further doses, unless I ask you to change them in some way.

If any old symptoms reappear, or any current symptoms worsen, this is a GOOD sign in most cases. DO NOT SUPPRESS them with other kinds of medication unless you are in real danger (which is a highly unlikely reaction to a remedy).

However, if you are on regular medication it is also important that you do NOT stop taking this. Once it is determined the medicine has acted beneficially, we can look at reducing such medication if safe to do so. Regular medication in this instance is NOT defined as other homoeopathic medicines which MUST be stopped.

Exactly how many doses and for how long entirely depends on what potencies you are able to obtain.

The other remedies I would seriously consider here is Androctonos and Alligator.

I can get Mercurius 30 in pills or liquid form today.

For the LM I will have to wait.

Can I start with the 30? And if so, how many doses?

Yes you can do that. What I would like you to do with 30c is actually just take one dose to begin with, wait 3 days and let me know what has happened. IF there is no strong reaction, I will get you to take 2 more doses, one each day. I want to test your sensitivity and reactivity first though with a single dose.

A particular remedy can have a very different effect on you though, especially if it is prescribed for the right reasons. I just want to go cautiously in the beginning, although it is good to know you probably aren't overly sensitive to remedies as a general issue.

I have taken a dose of merc. 30 3 days ago.-No signs of aggravation, no changes.-Do I go ahead with 2 more doses?And how long do I wait after ?those.

Dear Evocationer: As I told you I have been had almost 25 years of homeopathic treatment.-And the practitioner I have consulted used to prescribed by synonymous names generally; and today I found that I already took Mercurius sol. under the name: Hahnemanni, not long ago.It was in the M one dose and watch and waited for two weeks.-Please correct me if iam wrong about the names, I read it in a book at the pharmacy today.
Sorry I didnt mentioned before.-I really appreciate your help!! and I would like you to continue with my case.
I will copy a list of the remedies I ve taken if is ok with you.

I took one dose of the 1000c and there was no changes at all.-This was the last year and my symptoms didnt change since the beginning of my homeopathic treatment.-

Just in case it was a potency issue (too high and often people will feel no effect) we will first push the 30c. If nothing happens then I will change the remedy.

2 more doses, one each day, no pause between them.

I am less convinced of this remedy now that I know you have had it before. However, it is always nice to see that we homoeopaths can work along the same lines and see the same things.
