Anxiety. Pls help any experienced doctor

I have anxiety disorder. My symptoms are given below:
1. Fear of People
2. Nervousness in crowd
3. Avoiding social situations that make me anxious
4. Trembling of hands all the time
5. Heart rate increases all the time in social situations
6. Feels nervous when people looking at me
7. Inability to relax in social situation
8. fear of making mistakes
9. Can not take meal in group of friends
10. Anxious before going to big event like marriage, exam, temple etc
11. Fear of meeting new persons
12. What if” fears about situations far in the future
13. Can not make eye contact while talking to anyone
14. excessive self-criticism
15. Pounding heart beat
16. Stomach upset and diarrhea before going to marriage, exam, meeting new people
17. Speaking specific word or phase many times
18. Checking alarm many times
19. Checking doors many times
20. Checking books many times
21. Confusion, muscle tension, weak memory
22.fear of Asking questions or giving reports in groups
23. Can not use public toilets
24. Fear of talking on the telephone
25. Sadness and heavy head due to depression
