Tight foreskin while erection

Laziness is there and highly emotional.
Cold weather prefers.
Prefers vegetarian food,also eat chicken and egg.
From 13 years back masturbation was there (daily 2 or 3 ) times.recently it decreased to weekly once.
Issue:
Gynecomastia is there and pennis is bent left and foreskin can't retract back when it is erected and heavy pain while try to insert in vagina.
Pennis small in size,pink in colour.
Tried to do sex intercourse with a lady before 6 months back but due to pain in pennis can't proceed further.
