I am suffering from excessive pre-cum and PE and my penis not erect during penetration :/

I am suffering this from last 2 years, i tried many homeopathic doctors and medicines but nothing worked.

Note :
(1) - i feel extremely nervousness.
(2) - in a week 2 or 3 times my body vibrates like 1-2 seconds, don't know what is this :/

I ejaculates in 30 seconds.

My penis never erects when i try to do sex, i tried everything to get erection but nothing worked but on other time my penis erects like a hard wood, even at daily morning it erects.
When i force my penis to enter her vagina then suddenly it ejaculates :/

Daily automatically pre-cum releases, even it happens when i talk with my girlfriend but when i meet her then my penis crosses his limits and starts to releasing excessively and it sucking my body in ultra level, i am getting skinny.

I stopped masturbation 3 years ago, on that time my ejaculation time was good, everything was good and normal but after stopping that the issues started to happen.
Now my penis looks like micro penis, its shrunked.
Now my penis erect only 3cm :(
