Urgent help, Dementia , loss of balance, with long term high Bp

My father age 75yrs, has recently had cataract surgery, and his suddenly deteriorating , he is using a walking stick now, bcoz he is toppling over to one side without it, he has been becoming more & more forgetful over the years, sleeping often, can’t drive anymore, says he feels dizzy, is forgetting basic routes, forgets basic things, doesn’t talk to his family, talks to visitors, and May end up giving away private info,
He has had high BP since his early 30s, has diabetes since 10-12 years, which he took syzium jam for initially for years before starting metformin, some years back, don’t remember exactly when, was in denial that he had diabetes.
He falls bcoz of loosing balance or dizziness, and injures himself.
he is very thin and underweight, irritable, doesn’t listen to his wife or children for eating or walking , sits around whole day, and says he can’t walk, because he is so busy he didn’t get time. Has no activity nor hobby. He has stopped almost things he used to do.
He had an MRi, I can get the details for it, I think it’s vascular,

Please advise, we are quite worried for him and want him to be well.
