Anal Fissures or Fistula

I am 45 yrs old male suffering with Anal fissures or Fistula, I dont know what it is as on day because earlier problem of fissures was there and whenever it got aggrrevated I used to take Nitrum Acid prescribed by Homeopath Doctor which used to give me relief. But recently I am feeling some thing protuding out or feeling of part of anus is swollen specially after passing of stool. Hardly any pain, only bit of burning which reduces in 2-3 hours after defecation.
Please tell me if it is fistula or some thing else.
Also advise me some medicine for this.

I am 52 yrs old and have had 3 surgeries over the last 25 yrs. Each one about 5 yrs apart. The last on only about 2 yrs. I do not want to have to have surgery again. The last flare up lasted about 4 days until it ruptured, the pain on the last day was almost unbearable. It ruptured yesterday and is still draining. Please help I don't know what to do to keep this from coming back.

No there is no grape like protruding out just a hole on the surgery scar. I have bought the following but it has not arrived yet.
