Neuromuscular Jaw Disorder. here are my current diagnosis

After been to several Dr and Dentists. They concluded the following diagnosis
Neuromuscular Jaw Disorder ( TMJ / TMD )
My current complaints:
Headaches
Pain in and around the jaw areas
Ear Pain. If i try to use cotton swab it wont go in. Ear Holes are very stiff.
Ringing in ear, sometimes
Sleep Disturbance

i really need a treatment for this problem. Its chronic.
Here Dr gave me rough estimate of $15000 dollars and no insurance covers Orthotics. Plus, there is no guaranty that Orthotic will cure the problem. ( Per Surgeon )

I have teeth upper jaw, right side, 5 years ago have had root canal and taken cap,as there was a deep carriage there. But atfer two years it began pain and whenever, i take hecle lava -3X or 6X pain reduced. For this i also feel pain in ear, head, and some un-expressible feelings. Now i am suffering the same. which medicine can i take?
i posted in this thread coz i need your suggestion if i would post another thread , then i dont know whether i would get you or not.
