Hot, red, inflammed varicose vein in left leg

I've had large distended varicose veins in both legs since a teenager. I'm 56. I had them removed about 20 years ago. The left leg never came good, with swelling at the top of the calf and poor blood flow. I now have a hot red lump under the skin on the inside of my calf, which I suspect is an inflammed bunch of vein scar tissue. Any ideas?
