This is just a forum. Assume posts are not from medical professionals.
If you would like me to take your case I have a different intake form, as this one you have used has some fairly severe limitations.

GUIDELINES FOR GIVING HOMOEOPATHIC CASE INFORMATION

It is important to describe all your problems in as much detail as you are able. One word answers and short sentences are not particularly helpful. Discuss each problem one at a time, providing (as a minimum level of detail) the following information.

1. What exactly happens?
2. Describe all sensations and pains. Each pain or sensation should be described in such a way that allows us to imagine having the same pain.
3. What causes the problem to get worse after it has started occurring?
4. What creates some relief for the problem?
5. What triggers the problem into occuring?
6. What time of the day or night does the problem occur?
7. When did the problem start? What was happening in your life at that time? Did some specific event or treatment take place just before the problem started?

Move from one problem to the next, doing the same thing. IT IS VITAL THAT YOU GIVE A COMPLETE PICTURE OF YOUR HEALTH BY PROVIDING ALL PROBLEMS YOU HAVE, EVEN IF NOT CONNECTED TO THE MAIN ONE, AND EVEN IF YOU CONSIDER IT OF LESS IMPORTANCE. You should address each problem separately using the above 7 questions as a guide.

As well as this, please describe any traumatic incidents that have taken place in your life. Discuss anything that has had a lasting impact on you mentally, emotionally or physically.

Discuss the way that you manage or deal with your problems, or any problems that occur in your life.

Discuss any patterns you have noticed in your behavior especially concerning your disease.

Discuss any part of your life where you feel stuck or unable to change and grow, especially where this occurred around the beginning of your disease, or as the disease evolved.

Describe your childhood and the kind of environment you grew up in, with reference to your relationships with your family, your school experiences, and any serious childhood diseases.

If your earlier discussions have not mentioned these already, please describe:

1. The specific foods that you crave (not just like) or hate
2. The specific drinks that you crave or hate
3. What your sleep is like
4. How the weather and the temperature affects you
5. What kinds of things in the environment you are particularly sensitive to
6. What your general level of energy is like
7. What your level of sexual energy or desire is like
8. Describe your menstrual cycle

Two problems--
first is anxiety/social problems - I'm shy person. I'm more comfortable in a one-on-one setting (unless the other person is an authority figure-boss, interviewer, teacher) but as soon as a third person is involved, i get flustered when i'm expected to speak..My face and chest get red like I'm embarassed, but i'm not. I have a hardtime speaking up, my tongue feels swollen, and i'm at a loss of words, and sometimes have shortness of breath, like i forget to breathe. I also have a hard time making eye contact.
second is skin problems- i'm not sure if any of these are related, but here's everything. I have dry skin on my body, but my face and hair is oily. i often have flakes in my hair also. i have breakouts on the bottom half of my face (lower cheeks and chin) and sometimes on my chest. but not on my forehead/nose. i also have excessive hair (mostly light, but a few dark hairs) on my face and thick arm hairs as well.
2. Describe all sensations and pains. Each pain or sensation should be described in such a way that allows us to imagine having the same pain. I do not have any pain. The only sensation is the feeling of warmth in my face when i can tell my face is getting flushed.
3. What causes the problem to get worse after it has started occurring? once i feel my face getting red, then i feel embarassed about that, and feel it getting even more red- then i'm just ready for the conversation to end so i can stop feeling like that.
4. What creates some relief for the problem? birth control has helped my acne in the past, anxiety problems-- i loosen up when i have a couple drinks or when the spotlight is taken off of me.
5. What triggers the problem into occuring? talking with an authority figure, or in a group setting- even if it is family, or a group of friends
6. What time of the day or night does the problem occur? anytime of day, just depends on the situation
7. When did the problem start? What was happening in your life at that time? Did some specific event or treatment take place just before the problem started?i have always been a shy person, even as a child. but the problems has gotten progresively worse as an adult. i think in college is when i first noticed it- i had a speech class and was comletely terrified to get up in front and speak. also would not ask questions in any of my classes because it meant everyoen would be lookign at me, and i was afraid of looking dumb.
acne problems- started when i was about 17 years old- before then i had clear skin. in college i started takign birth control pill and this got better, but now it is worse again, and i dont understand why i have acne as a 28 year old adult.

my parents divorced when i was 12, my dad remarried when i was about 17 and i have not had a relationship with him really since i was 12. other than this, i think my live has been pretty traumatic-free
Discuss the way that you manage or deal with your problems, or any problems that occur in your life. i do not have many close friends. if i talk to anyone about problems, it is with my husband, but typically, i just work them through in my head. it helps for me to work out a problem with a future plan for making it better.i typically write it out on paper.

Discuss any patterns you have noticed in your behavior especially concerning your disease.
even with daily contact with my boss and being on the phone at work, i still have these shyness/flushing/anxiety problems, instead of it getting better, it seems to get worse. i can think it through logically in my head and know nothing BAD can happen just from a conversation, but still when the time comes, i pull back and the shyness comes out.

Discuss any part of your life where you feel stuck or unable to change and grow, especially where this occurred around the beginning of your disease, or as the disease evolved.
when i graduated from college, i got a job that i did not want, but it was all i could find. i've been stuck at teh same company since then, and feel like a failure because i have interviewed several times in the last 6 years, but have never been offered another job. i feel like i will be stuck here forever, if i don't get these issues taken care of.

Describe your childhood and the kind of environment you grew up in, with reference to your relationships with your family, your school experiences, and any serious childhood diseases.
i had a happy childhood- close with my mom and dad both until their divorce- then my relationship with my dad pretty much stopped. i was always popular is grade school and highschool- had several boyfriends and friends. i was always kind of shy in a classroom setting, but in college i became more intraverted. i have been with my husband off and on since my freshman year of highschool- and we married in oct 2010. we have a very solid relationship, but do feel that being in a relationship through college made me miss out on developing college friendships. but also part of this is that i was too shy and nervous to talk to new people.

If your earlier discussions have not mentioned these already, please describe:

1. The specific foods that you crave (not just like) or hate- i crave chocolate, and steaks, there are no foods that i hate
2. The specific drinks that you crave or hate-i crave sodas alot, do not like protein drinks at all.
3. What your sleep is like- i sleep well, i have a lot of dreams, and usually remember alot of details the next day.
4. How the weather and the temperature affects you- in the winter my skin gets very dry, and then better in the summer-- but still have the issues with oilyness in all year round. mood-wise, i like cold to mild temperatures best-- i do not like hot temperatures
5. What kinds of things in the environment you are particularly sensitive to- bright lights make my eyes burn and water. i think i also have very sensitive hearing and smelling. i'm easily annoyed by someones voice or laugh.
6. What your general level of energy is like- it goes in spurts. sometimes i will feel good for a few days and will exercise. then other times i feel so tired and feel like i just need to sleep a few more hours in the morning.. then come home from work and want to do nothing
7. What your level of sexual energy or desire is like- lacking. i will go a few weeks without having any desire, then when i finally do have sex, i usually want it again for the next few days. then will go another 2 or 3 weeks without caring about it again.
8. Describe your menstrual cycle - i have one every month, but usually they are very light, and only last about 2 days

thank you!!
