azoospermia possible obstruction Dr mahfooz help needed
salaam,I am happily married for last 3 years and trying for the baby but no luck. Recent test have shown zero sperm count.
Result as below:
Age 34
Volume 2.8 ml
Number per ml 0
Total count 0
Viscosity Normal
Agglutination N
Viscosity Normal
pH 8.5
Round cells per 100 ml 1.2 million
after this the Dr has suggested me to do the scrotal scan. In the scan they haven't found any thing except size of scrotal are bit small than normal, according to doctor every thing is normal.
LH and FSH level are also fine. The Doctor is suggesting to go for IVF but my wife is not ready yet and I am also bit reluctant.
I am already acting on doctors advice by not taking warm bath and wearing loose under garments.
My sex life is fine and have no issues the only thing I have observed I often get release before my partner do and after some time of erection some transparent fluid starts coming. which according to Doctor is normal.
One more thing I feel so much hot during sex I often start sweating, I am not able bear hot weather at all. as soon I start some thing physical I will start sweating heavily.
The only surgery in my life I have is Appendicitis. The only physical thing I can think of is my stomach is not that strong often I have Diarrhea.
Dr Mahfooz can you please advise if this azoospermia which according to Doctors is due to some obstruction (which was not evident in the scrotal scan) is treatable.
I would be really thankful if you can please advise some medicine and let me know if I have to abstain from something during this treatment.

Thanks for your quick response and sorry for the late reply as I have to confirm history from my parents and Doctor.
Yes I had mumps at the age of 7 or 8. I have an operation of Tonsillitis at the age of 5.
I have appendicitis operation at the age of 15.
The results of the other test conducted to prove the azoospermia is as below:
Blood Test

1- Karyotype: 46,XY
--------------------
Chromosome analysis of the cultured blood sample from this patient has shown as apparently normal male karyotype.
No sex chromosome mosaicism was detected in 30 cells examined.
2- Molecular Testing for Cystic Fibrosis
-----------------------------------------
DNA from this patient has been tested for the common delta F508 (p.Phe508del) cystic fibrosis gene (CFTR) mutation and other mutations as below.
No mutations were detected by our test.
3- Testes Sonography
--------------------
Both testes display a homogeneous appearances with normal vascularity. Both testes are slightly smaller than average (right = 17 x 40 x 26mm, left = 37 x 17 x 25mm). Both epididymides are normal in size with no focal abnormality. No varicocele or hydrocele.
4- LH and FSH
-------------
LH 9.2
FSH 8.0
Oestradiol 109
Testosterone 9.1
prolactin 208
Summary given by Doctor after all these tests are:

Patient has normal Karyotype, his Delta F508 assessment for Cystic Fibrosis is normal and his endocrine screen is essentially normal apart from a marginally raised LH which is of no significance. The ultrasound of the testes are also normal. The patient has infertility secondary to azoospermia and therefore will require surgical sperm retrieval.
Also as mentioned earlier my sex life is normal except some times it feels like I am ejaculating pre-maturally.
I have tried to cover every thing best to my knowledge, if you have any questions in regards to any tests done for confirming azoospermia please raise this so that I can get answer for you.

Please take the medicine as per following protocol:
Thuja-1M ( 5 drops once in 15 days mean after two weeks)
Paratodnium-1M (5 drops after 2 weeks) and
Damiana-Q 15 drops three times a day)
continue this protocol for 3 months and give progress.
