Advice needed for 7-year old boy with sleeping and hearing difficulties

I have a 7-year old boy that has had problems falling asleep for several years now. Goes to bed at around 8-8.30 and lies awake for 1-3 hours before falling asleep. Complains of being cold at night and often wants an extra sweater or blanket. Complains of restless legs a lot, says his legs cannot be still for very long during night (every night). Also sometimes wakes up in the middle of the night from severe pain in the legs, usually in the knees, but this happens only once per week or so. Seems to be overtired because of this and has minor difficulties in concentrating during daytime.

Complains of impaired hearing due to the feeling of a plug in both ears. Wants to clean his inner ear a lot and has more earwax than normal. Occasionally complains of some minor pain in ears. Speaks rather loud. The impaired hearing does not seem to be caused by too much wax though, but more due to some internal pressure (as from diving). At one occasion he exclaimed 'my ears just changed - do I speak this loudly all the time?'. The effect lasted for about a day or so.

Judges himself quite hard in his intellectual work and in playing games, but this has improved lately. Difficulties in accepting defeat or things not turning out the way he wants them to. Has had difficulties in accepting praise from parents, but this has become a lot better lately.

Has begun to twitch his eyes lately as well as sometimes putting his fingers in his mouth from time to time, but not to bite his nails or sucking on them, more like just touching the lips. The eye twitching kind of looks like as if the eyes enlarges and will almost pop out. But he does not complain about this though.

As a newborn, he had severe cholic for 6 months. Unusually fast in learning to walk and talk. Highly intelligent with a practical and creatively technical way of expressing himself.

Any advice would be much appreciated!
