Congestive Prostate - Spermatorrhoea Urgent help please

I am 26 years old , I started masturbation at the age of 15 accidentally.. Earlier i didnt know what this act is actually. but as time passes i became addict. Now almost I left this habit.

But this habit effected my health so much. I m worried now my main concern is that when ever I pass pee or take shower there is semen or thicker white liquid discharge before and after urine. This problem destroy my health and I feel weakness whole day and frustrated want to get rid this disease as soon as possible. I search on internet is these symptoms are for congestive prostate or spermatorrhoea?

My symptoms are..

Less Eriction than before.
Sometime pain in abdomen or lower back
Very Early ejeculation while mastubrating.
Hair Loss.
Belly Increase
Eye sight effected.
Now My penis is shrink & very weak
Pain in my joints
Leakage of semen or white liquid in urine
Dark circles under eyes
Always tired
