Small Joints Pain

I am having pain with swellings mainly in feets. Toe, fingers, above heels.
It has become almost a permanent feature for the last one year. Every 2nd week it flares up with very unbearable pain that I can not even walk or wear a slipper.And it stays for a week and then subsides slowly. But again after 15-20 days, it comes back.
I have a high uric acid tendency but I am taking regular medicines (allopath)for almost last one year.
I am having some other problems also like Diabetes since last 13 years, Hypertension for the last 24 years. Albuminaria for the last one year.
With all these, I have become highly depperessed.
I am 55 years male.
Please help me.
