Feel Sleepy and Lethargic at daytime difficult to live normal life

Dear Dr,

My age is 30 years.
I always feel lethargic and sleepy at day time (mostly 11am - 4pm),
nighttime sleep is about 8-9 hours.
I feel very difficult to concentrate and comprehend and at times talk properly during that period.

Body energy does not feel much of a problem its most mental energy . I look bodily fit.

I feel that mind is generally not alert fully and I lack its presence most of the times.
Because of this i forget things also. Most of the times I do things without fully alert mind.
Alertness problem is lesser in evenings and night.

I have social phobia also. get nervous easily in meetings . not able to collect my thoughts and speak. at that time I am even not aware clearly what i have to speak and what I am speaking.

I try meditation also. sometime when I do meditation in morning , I feel day time lethargy more. may be because I have utilised my mental capacity.
But meditation helps a lot in giving alertness and clarity for a short period although.

I am experiencing this problem from a very long time 8-9 years.


my gulucose tolerance test was aslo not good.
Result
Fasting 89 mg/dl
After 2 hrs 146mg/dl

I need some experts advice .Please help.

Please find my answers below.

ANS. 30 , Male , wight 84,Body built is mesomorph,well built and face apperance is dull,tired,heavy and dark circles around eyes , Country india, occupation is Software enginner

2. Main complaints and other associated troubles.
a)Where is the trouble; The exact locality of the complaint like hands,legs etc; duration of trouble.
ANS. Mind , its slow,lethargic and sleepy, Not able to concentrate for more than a small time.
Sometimes during conversation I loose the track. I generally do things without the full or even presense of mind. Sometime I stammer and Many times I find diffuculty of finding words while speaking.
During conversations or discussions, mind feels very slow and not able to get ideas , not able to process things fast . or do runtime small calculations or understands the tings as whole. But later when I sit alone I slowly understands and comprehend the discussions , if i haven't forgotten them . This is the reason I tend to make the conversation short.
If I have to speak in a meeting. I get a sensation of cold in meeting room before the discussion begins. and I get tense before my turn comes up. heart pounds fast and heavy.
Sometimes mind gets blank and its difficult to collect thoughts. and if someone asks a question which is not what I have thought of. I become blank. mind doesnt helps .

While reading books and articles, I find it difficult to comprehend as whole what i have read till now. I do understand the individual sentances but imagining as whole is difficult but not impossible.
During reading casual articals , I tend to skip lots of sentences in between. I try to search what I need.

And all during rare days when my energy level is good. I do not feel many of the issues listed above.

ANS. If I understands question correctly. pain feels like burning
c)What are the factors that causes this trouble according to you.
ANS. I had depression from childhood and a history of daily masturbation which might have caused it.
d)Condition under which the complaint is reduced or you feel better like,cold or hot application,cold or hot weather,position as standing,walking,rest etc.
ANS. I feel cold weather is more sutaible for me and while i rest, I feel better.
e)Condition under which the complaint is increased like,cold or hot application,cold or hot weather,position as standing,walking,rest etc.
ANS. Majorly problem happens at day time. Or when I am exhausted. I dont find myself comfortable with hot weather.
f)Any other complaint any where in the body.
ANS. Digestions is not good. stomach gets upset frequently.
g)Onset time of troubles in detail, i.e which came first, after that what problem and so on.
ANS. The mind problem was existing since 10-12 years. but I realised this as problem 3 years back only. before that I used to take it as normal.
h)Treatment method adopted and its result.
ANS.

Ayurvedic treatment using ashwagandha.
2 year back
duration 3 months
Helped a lot in energy, and mind clarity , I used to think that I have recovered during use.
After 3 months I started feeling uncomfotrable. and felt drastic decrease in libido and erection quality. so stopped it.
After stopping it problems came back. but not as bad as they were earlier. libido is still not as before.
I had not so severe acne on face and back which got treated by ashwagandha.

3. History of diseases in family.
ANS. My mother has depression and issue with thinking a lot over small small issues.

4. Personal History.
a)About childhood.
ANS. Did not enjoy childhood. Had deppression and phobia of people. had low self esteem. which I have even now but not at that extent
b)Academic performance.
ANS. Average in studies. But did not study properly just use to mugup things to get passing marks. Did not understand the proper meaning of things.
c)Any major incidents in life and the effect of it on life.
ANS. No
d)How you are satisfied with your sex life, friends, family members, company etc.
ANS. I am not satisfied by sex life. It used to be far better . Dont have much expectations from family and friend.

5. Habits/Addiction.
a)Smoking, Alcohol,Sleeping pills, Laxative etc.
ANS. have history of smoking and alcohol for 5 years. Now I have stopped it since 6 years. After smoking and drinking alcohol I used to feel more of the problems explained related to mind and energy.
b)Masturbation and frequency.
ANS. Used to to it almost everyday from age of 11-13. or when I used to feel deppressed
Now it has reduced to 2-3 times a week.
6. How is your Appetite and Thirst.
ANS. appetite is good. I drink sufficient water in a day to make myself hydrated 2-3 liters.

ANS. I am vegetarian. prefer mildly spicy food . cannot tolerate spicy food in large quantity.

ANS. Sleep quality is bad. always tend to sleep on stomach. need window to be open during sleep otherwise feel suffocating. no need to cover body.
Sleep is restless and disturbed easily. after waking up in morning dont feel much relaxed.
Dreams always of running away from invincible ememies (ghosts) . when i kill them they comeback again. If I get a weapon to fight enemies (for ex Gun) it doesn't work.
If it works it doesnt kill the enemy.
In dreams I always have capability of flying. I can easly fly away anywhere.

13. Sweat
a)How much, what parts, staining, Odour.
ANS. Normally no sweats. but feets and underarms smells a lot.

14. Weather
a)Tolerance to heat and cold, dryness, humidity, weather changes, sun,
foggy weather, wind drafts, closed rooms, etc.
ANS. can tolerate cold, dryness , foggy weather.
cannot tolerte heat, humidity, closed rooms.
While coming out in day . feels uncomfort in eyes.

ANS. I feel care for my family would help them .

ANS. Memory is bad if mind was not alert when event was happening. difficuly in concentrating and comprehend

ANS. when i get attention in crowd, or when have to speak on some topic. I dont have much confidence .

ANS. yes I am hurt easily. I shake in emotional situation and cannot think properly and clearly until sometime. I just convinece myself do not carry on hatered and revenge.
I have jealesy sometimes if I see someone better then me. want to have those qualities in me.

ANS. yes easily. stupid mistakes of people around me. and if they keep on repeating mistakes. By shouting or talking arrogantly to them

ANS. Decisions are not well planned . Sometimes take decisions in hurry just by looking at one or two parameters. later have to regret.

ANS. I like to remain alone . or with someone who is very close to me.

ANS. It doesnot affects me much unless it become smelly. I am not organised.
u)How does failure appear to you?
ANS. Failure is a roadblocker for me. If some big failure comes it take days for me to come out of it. all sleep ,life mind is disturbed. I would dream also about the situation.
v)Are there any matters that you deeply dislike?
ANS.

ANS. I like mental peace. doing meditation it pacifies mind and mood.
x)Are you affectionate? How does others sorrow affect you?
ANS. Not much. I cannot empathize much. But I try my best to help them out.

ANS. I have fear of failures not leading a successful life.

ANS. desire to be at place where i think I am successfull. having a good health and mind.

its anxiety and depression dnt worry you will be fine, have confidence in yourself and faith in god,

take PLATINA 200, 2 drops in a tablespoon water, only 2 dose not more than that, not daily, 1st dose before sleep and next dose next morning after wakeup, dnt eat or drink anything 30 minutes before or after medicine,
{if pills then 3 pills as one dose}

tongue diagnosis:

Colour pink , slight teethmarks , white coating throughout the tongue which is thick in end and thin near tip.
Throat is dry, stale mustard like taste .
I see dark red dots on top of tongue near both edges.
trembling in tongue

There are non painful pimple like comings in side of tongue near start of it

3 tablets each 30 minutes before breakfast and dinner both, daily, chew it, dnt swallow with water,

rahul very good improvement,
you are needing nap bcoz your mind is wanting relaxation, so at that time do some deep breathing by closing eyes (5 minutes) and if possible take some nap, that will be best.

continue with mentat, kali phos and yoga,

take biochemic mineral cell salt,

NAT MUR 6X -- 3 pills 30 minutes before lunch daily

report after 20 days,

Dear Doctor,

since last two days I suddenly started feeling very tired and sleepy through out day even after sufficient rest.
So I stopped taking Kali Phos 6x from yesterday evening.
Not sure if that was causing issue. But Now I feel back to normal.

I still have to start Nat Mur from your last prescription.

Please suggest next actions.

9- confidence in meetings= Better, Some times trouble speaking clearly due to nervousness, but now I am more controlled, No hands shaking like before
Memory,concentration has improved.

One more update sir. two weeks back I went for regular health checkup there my blood pressure came out 140/80 .
In second visit it was the same. So doctor had some tests done. Test results came out to be normal.
