Excessive masterbation

Hi i am searching for a remedy to cure my negative effects of over masterbation. I am 23 now and have been master bating since I was 12 2-3 x's a day..I have slowed down in the recent years but can't help but feel constant fatigue..I have eye floaters, lower back pain, premature ejaculation of colorless semen whenever I get a prolonged erection, or think about sex..I can't last longer than 1 minute during intercourse. I need serious help! My prostate hurts at times after I ejeculate from masterbation, (a dull pain).. Please help!
