Severe brain fog, headaches and anxiety.

I am a 47 year old female. I've read the archives on the forum on brain fog just in case there was an answer so I wouldn't waste anyone's time. There were one or two posts that were close to my symptoms but no replies about what to do. Please, please if anyone can help I would so appreciate it.
I went through menopause when I was 40 years old and have had a history of a sluggish thyroid. (Recent blood work was: TSH 0.55 with the range 0.35-5.50). I also have a history of depression, endometriosis and gastritis.
The brain fog started about 3 months ago and has gotten worse. I can't remember things just seconds after hearing or reading something. For example, this morning I deleted an email and then decided I wanted to keep it. I immediately went to the trash email box but couldn't remember a single thing about the email I was looking for. Even now trying to write this, I have to struggle to remember what letters go into the words I'm writing. I used to read all the time and have a wonderful library of books but I can't read anything anymore. I can't comprehend what I read and feel easily confused.
Fatigue- I feel overwhelming fatigue all of the time. It's so bad I feel sick and like I have a hangover. I don't sleep well at night and never have. It takes me up to 2 hours to fall asleep and then I wake every 1-2 hours. I take 750mg of GABA, 100mg 5 HTP, 3mg of melatonin, 1600mcg folic acid, 20mg Cymbalta, and up to 5 tablets of Hylands Restful Legs. I've been tapering off of Cymbalta for 9 long months. I started taking the Hylands Restful Legs for restless leg syndrome two weeks ago. It's the only thing that has actually helped me. When I go to bed, I feel like I can't get comfortable, my mind won't shut down, and I feel hot, then cold, then hot, then cold. I do dream but can only remember some of my dreams. The dream I have most often is about pregnancy and/or babies.
I have little to no appetite yet have not lost any weight. I've gained 20 lbs in two years even though I've never really had a weight problem. No matter what I've tried, I haven't been able to lose the weight. I crave both salty and sweet foods. Because I have such a bad appetite I eat poorly and always have.
Anxiety- The anxiety started when the severe brain fog started. I feel anxious when I try to read anything or concentrate on something. I feel anxious when I have a task to do because I just can't seem to get anything done. I feel anxious when attempting any sustained mental activity including writing this post!
Headache- I wake up almost everyday with a headache. It starts as a band around my head and after 800mg of ibuprofen it will lessen to a shooting pain at the back of my head when I move my head. I clench my jaw when both awake and asleep usually not realizing (when awake) I'm doing it. When I have the headache at night it hurts to lay my head on the pillow.
I work from home right now and have a creative/craft business. I considered possible toxins but recently was away from home for two weeks and the symptoms stayed the same. Up until recently I have always been energetic, hard working and motivated. My business is finally becoming successful and its devastating to have these symptoms.
Medications/supplements:
Black Cohosh 40mg twice a day (started 3 weeks ago for hot flashes)
Ginkgo Biloba 120mg (started two weeks ago)
Folic acid 1600mcg at night for RLS
Melatonin 3mg at night
5HTP 100mg at night
GABA 750mg at night
Cymbalta 20 mgs at night
Ibuprofen 800mg usually twice a day
Tums & Pepto Bismal for gastritis several times daily
I also smoke a pack of cigarette a day and desperately want to quit but lately am craving them constantly. I drink coffee daily, regular in the morning and decaf in the afternoon. I also drink flavored waters throughout the day.

2. What other physical sufferings do you have in your body? Since this past Monday I have the brain fog but also headache,nausea, fatigue, apathy, lethargy.I gained 20 lbs in the last two years and have not been able to lose it.

3. What mental sufferings / feelings do you have associated with your physical sufferings? Anxiety and some hopelessness.

4. What exactly do you feel when you are at your worst? Describe the sensation in your own words. At my worst, I can't remember things, I can't concentrate or focus. Then I feel anxious because of it.

5. When did it all start? Can you connect it to any past event or disease? What was happening in your life just before these symptoms were noticed? It started approximately 3 months ago. I haven't been able to connect anything to it. I started reducing Cymbalta from 60mg to 30 mg but that was 9 months ago. 2 months ago I went from 30mg to 20mg.

6. What time of the day do you suffer the most? What time of the day /night do you feel most energetic and happy? I feel like the brain fog is bad all day and night.I think possibly the best time of day is late morning to early afternoon.

7. What are the things that aggravate your suffering and those that ameliorate the same? Example: time, hot or cold application, pressure, rubbing, washing, eating, tight clothing, sweating, walking, climbing, stool etc. Nothing seems to aggravate the brain fog, its just constant. Nothing to seems to help it either, not sleeping or eating.

8. Do you think your sufferings have direct relation to any particular external factor or are it something to do with your own biological changes? No to external factors and the and I don't know if there are biological factors.
9. When do you feel better, during hot weather or cold weather, humid or dry weather? I feel better during dry, hot weather. Or warm weather ideally.

11. What are your fears and do you dream of any situation repeatedly? My fears are lack of making ends meet and I dream very often about being pregnant or having a baby.

12. What do you crave in food items and what are your aversions? Usually I crave meat, salty foods and sweets after eating. Since Monday, I have an aversion to everything.

13. How is your thirst: Less, Normal or Excessive? Normal.

14. How is your hunger: Less, Normal or Excessive? Less.

15. Is there any kind of food which your body can’t stand? No but B vitamin supplements in pill form make me sick.

16. Is your sweat normal or less or more? Where does it sweat more: Head, Trunk or Limbs? Normal, and I sweat more on my head.

17. How is your bowel movement and stool type? Do you have any abnormal smell in the urine? I alternate between diarrhea and constipation.

18. How well do you sleep? Do you have a particular posture of sleeping? I don't sleep well and never have. I have a lot of trouble falling asleep and then trouble staying asleep. I also have restless leg syndrome. I usually sleep on my side, typically my left side.

19. Do you think you are able to address your libido in general? Would you say your drive is low, normal or high? Normal.

20. Do you have any strange, peculiar or unusual sensation, thoughts or feelings? How are you different from others? I don't think I do and I don't think I'm different in any significant sense beyond the usual differences.

21. What medications have been taken earlier by you to treat the diseases and do you have any particular symptom surfacing after the medication? I haven't taken anything for the brain fog nor for my recent physical symptoms except for ibuprofen for the headache.

22. What major diseases have run in the family in the last two generations both sides? Depression, alcoholism (I don't drink), stomach problems.

23. Describe your overall appearance with respect to your BMI, skin type, muscular or flabby etc. I'm 5'7" and 130lbs. I'm unhappy with the weight gain I've had and I would say I'm flabby. I'm Irish and have fairly light skin.
24. What major diseases have you had in your life and when. Please write them in a chronological manner. Depression, on and off most of my adult life. Endometriosis and hypothyroidism most of my adult life.I went through menopause at 40 years old and have not had a period in several years. I am still experiencing a lot of hot flashes.

Will come back to you. Seems your case is curable if you gradually taper off your dependence on cigarettes, coffee and other drugs you are taking as supplements. However, for your TSH, if you are taking thyroid tablets, this should be tapered off very very slowly based on your response to homeopathy.

Please take 2 doses of LACHESIS MUTUS 200C, on a single day, 12 hours apart.

One dose would be 3 drops of remedy in about 10 ml of water slowly sipped up in empty stomach and clean mouth. No food or drinks one hour before or after taking the dose.

You should not have any pain relievers or anti-biotics for the next one week.

I expect this would help you to get good sleep, cure morning headaches, reduce hot flashes at night and possibly elevate your moods.

Let me know the changes you feel in the next 10 days. Based on your feed back the next course of treatment could be thought of.
