Constipation, bone joints clicking sound, possible spermatorrhoea

I have over 15 years of constipation. Can't empty bowels fully, stools come in pieces or very little, flatulance, stomach remains bloated like gas balloon.
Urine looks like as if colourless honey is poured into water. Sometimes cloudy. Important thing is when I do excercise or physical activity I lose body mass very quickly possibly muscles aswell. I remember doing a heavy delivery job that included walking, lifting etc I started to lose weight even I started to lose mass around penis and eventually I couldn't control urine. After leaving that job and resting alot made it better.
Also clicking sounds from joints including wrist, finfersØ knees even jawbones.
I might be releasing sperms, proteins etc in urine as I feel body and sexual weakness aswell.
What test do I need to take to check discharge (jaryan).
Recommend the treatment please.

Don't eat or drink anything within 30 minutes
