Fear/aversion to water in ears and limb flail

This is my first time here...and am so due to a search on this topic... I have 5 year old identicle twin boys, and have used homeopathy on them in the past for colds, earaches, and behavioral issues (they were put on the autism spectrum at 18 months old and were 'removed' from it in the spring of 2011 after two years of TFT and homeo)
I have noticed in the last year one of the boys has had an aversion to his ears and face getting wet-submerged..as a baby he was 'ok' with it during bath, and swimming (he swam a lot!) now he is falling behind his brother and his peers in swimming classes and other such water sports - although he does like water and plays in it, loves bath time and is generally ok when washing hair
I have noticed in the last 5 months or so, be has also been having odd arm flails...they have 'grown' to jumping or leg kicks...they seem primarily to be when he is overly excited about something (or that is what it seems) in the moment he also seems inhale deeply and exhale when done the flail...this lasts perhaps 1-2 seconds...
I seriously believe these actions may actually may have to do with him holding back his bowel movements (he often holds for 2-3 days) as they seem to subside after a good size

He is a very social and happy boy, extremely bright, plays well, and can also be very emotional too.

I'm not sure if these can be treated homeopathically but if so any ideas would be truly appreciated!! Thank you

What medicines was he given previously? Do you know which ones actually helped him?

ID: His name is Austin, 5.5 years old, Identicle Twin, 45lbs, 46' tall, lives in Ontario, Canada, currently winter but mild, live in the country.

Complaints: Aversion to water in ears, and sporatic limb flailing (seems involuntary but mostly when excited or anticpating something)

Non diabetic, desires sweets and any 'grain' type food (bread, granola, cereal, etc). Has a healthy diet and is restricted to amount of wheat he consumes (had been on GF/CF diet two years ago with no signifigance)

He has primarily been on Sac.Alb.1M and some others which for other issues (colds etc) with success for other issues (behavioural) he has not been on anything for these two issues.

Family background: all healthy, no major allergies or health issues, all grandparents and other family members still living.

Food: again, he loves what most 5 year olds love (if he could he would eat chicken fingers and fries every day) He enjoys salads, meats, fish, some fruit...he craves 'unhealthy' food (would prefer a granola bar or crackers over apple slices and cheese - yet will eat the healthy without a fight!!) he craves breads crackers and cereals etc.

Behaviour: very social,curious,loves to travel - explore, talks aLOT, loud talker!shouts to be heard most often, self centered (world still revolves around him), impatient, not overly empathetic except with his twin, frustrated, cries a lot and can be whiney and dramatic when doesn't get his way, bossy, pushy, very happy and very comedic, excitable, likes to be around people, likes to be alone too, 'lazy' as in he would prefer to watch TV than go outside to play, yet once outside enjoys himself, very plan oriented, schedules/plans out the day, very time oriented, likes to know what is coming, when, who and how, very loving with parents and other loved ones, likes to touch people, be close (almost in peoples personal spaces, loves music, tv, computers, (although he doesn't get much time on that)

In homoeopathy all issues are seen as part of one larger issue, and so one remedy is applied to it. If Sacch worked so well before, it is probably worth repeating it to see if it helps again (unless these problems were present during that treatment plan and remained unaffected).

At the very least, if his state truly has changed, the Sacch will emphasise what needs to be looked at.

In clinic this is the way I would approach a case that had already had some success.
